  2. Ontario Health Team Ontario Health Team - - Overview Overview An OHT is not a new entity it s a new way of working together. The LLG OHT is a partnership of more than 50 health, community and social service organizations from across the Counties of Lanark, Leeds and Grenville. Our goal is to work together to improve your healthcare experience in the region, focusing on 4 key areas known as the quadruple aim: improving patient experience improving population health outcomes improving care team well-being reducing costs

  3. Lanark Leeds, and Grenville OHTs are not restricted to geographical boundaries like the previous LHIN structure Partnerships are formed naturally where the services and need is present. Our partners primarily are located in the tri- county area.

OHT Working Groups and Committees OHT Working Groups and Committees Connecting Patients to Primary Care Full time intake coordinator working to support attachment of patients to primary care providers. Virtual care pilot project with UCFHT, ConnectWell, RCHS, and NPLC Connecting discharged persons who were admitted to hospital Navigation Project- province wide to determine navigation resource gaps and opportunities

OHT Working Groups and Committees OHT Working Groups and Committees Connecting Mental Health & Addictions to Primary Care Survey conducted reaching close to 500 community members aimed at evaluating the current state Forum held to discuss the current state of MHA Plan: to develop an implementation plan for connecting MHA services to primary care across LLG

OHT Working Groups and Committees OHT Working Groups and Committees Digital Health Working towards establishing privacy governance for the OHT Supporting online appointment booking pilot with Telus / Ocean for some Primary Care Providers Joint tri-OHT digital health table across FLA, LLG and HPE OHTs aimed at further aligning digital health strategies and opportunities. Future: On line patient portals and system navigation

  10. Lived Experience Advisory Network Lived Experience Advisory Network We are the voice of those who have accessed services/healthcare within the catchment area of the LLG OHT. The patient voice is essential in the delivery of person-centered care Persons with lived experience partner with healthcare/social service providers to improve the experience and to achieve the best possible outcomes for all.   11. Lived Experience Advisory Network Lived Experience Advisory Network Comprised of 12-15 lived-experience advisors from across LLG. Identifies opportunities to improve the accessibility, safe delivery and quality of care Partners in developing, planning, and implementing continuous quality improvement initiatives. Actively promotes and creates new and unique opportunities for communication, collaboration and partnerships among patients, clients, caregivers, families and community members and health providers. Promotes the inclusion of all voices in health and well- being journey.

LEAN Contd LEAN Cont d Representation of diverse individuals from across all of LLG Wide range of lived experience from many areas in the healthcare system (seniors, palliative care, mental health and addictions) Meeting since Fall of 2020 LEAN is represented across the entire OHT Collaboration Council, Multi-Sectoral Tables, Working Groups and Committees.

Current Focus Current Focus Caregiver ID Project Working group developed Goal is to have a standardized approach across LLG for caregiver presence Several groups have volunteered to start this project Led by Suzanne Rintoul (member of LEAN and ED for Seniors Therapeutic Centre-Perth) Transitions in Care New project Just in development Advocacy Empowerment

  14. Recruitment Recruitment Recruitment process is currently being discussed. Requirements for membership include: A person with lived experience accessing LLG OHT programs and services. Can represent persons with lived experience effectively and work collaboratively when engaging with all stakeholders, including members of the community. Can commit 2-6 hours to the Network per month. Ensure compliance and respect for privacy and confidentiality requirements. Demonstrate compliance with Rules of Engagement
