Our ambition Our ambition Our ambition is to improve the lives of everyone living across Sussex now and in the future. We want local people to thrive to be the best they can be; to be healthier and feel supported; and have the best possible services available to them when and where they need them.
Why we need a plan Why we need a plan We need to address and improve a number of critical issues across our Sussex Health and Care System Growing need for services, due to increasing and ageing population, impact of pandemic and wider determinants of health Health inequalities, particularly among those most disadvantaged. Disjoined and confusing services Recruitment and retention of staff Untapped potential of digital technology and information Better use of finances
We want to: We want to: Help local children and young people to start their lives well by focusing on improving their health, support and care, as well as their parents and carers Help local people to live well by supporting people to maintain their own health and wellbeing and to look after themselves better Help local people to age well by supporting people to stay healthy and live independently for as long as possible, reducing falls and improving their experience at the end of their lives. Help local people get the care or support they need when they need it by improving services available to them. Help our staff to do the best job they can in the best possible working environment by doing more to support their own health and wellbeing and to promote opportunities.
Long Long- -term Improvement Priorities term Improvement Priorities Many of the issues and challenges we face can only be addressed and improved with sustained long-term change. We are focusing on three areas: Integrated Community Teams to join-up heath and care services around communities to better meet the needs of local people. Growing and developing our workforce by developing a one team approach to allow staff to work together across different areas and organisations, recruiting more new talent and getting the best out of our existing workforce. Improving the use of digital technology and information to help join-up services and care, and make it easier for people to access services and manage their own health.
Immediate Improvement Priorities Immediate Improvement Priorities We are taking immediate actions this year to improve access to services in the short-term. We are giving particular focus to four areas that public feedback and performance data has highlighted need the most improvement: Increasing access and reducing variability in Primary Care. Improving response times to 999 calls and reducing A&E waiting times. Reducing diagnostic and planned care waiting lists Accelerating patient flow and discharge in hospitals.
Continuous Improvement Priorities Continuous Improvement Priorities To support all the immediate and long-term improvements we need to make, there are four key areas that need continuous focus and improvement: Addressing differences in health ( health inequalities ) that exist across our different populations, particularly our most disadvantaged communities. Addressing the mental health, learning disabilities and autism service improvements that we need to make. Strengthening clinical leadership to allow us to make improvements to both health and care services and the health outcomes of local people. Getting the best use of the finances available so we can invest in services and make sure we are working in the most effective and efficient way.
Health and Wellbeing Strategies Health and Wellbeing Strategies Our ambition is built on the three Health and Wellbeing Strategies we already have in place across Brighton and Hove, East Sussex and West Sussex. These use local evidence, data and insight to set out the priorities for improving health and wellbeing of their populations, responding to the distinct issues and challenges. We are also strengthening our health and care partnerships across Brighton and Hove, East Sussex and West Sussex known as Place-based partnerships which are made up of the local authority and local NHS organisations working together with wider partners to oversee and lead the delivery of services at a local level.
Benefits for local people Benefits for local people By delivering this plan and partners working closer together, local people across Sussex will experience a number of different benefits over the next five years. These include: More people being better supported to manage their own health and wellbeing so they can stay healthier for longer and will be better supported to live a more fulfilled life and retain independence when they do become ill. More people and carers experiencing support and care that better meets their individual needs and is better suited to the community they live. More people having better access to services and experience more seamless care that is more focused on preventing them becoming ill. More people experiencing shorter waits for treatment.
More people being cared for closer to home and those who do need inpatient hospital care will be able to return home quicker. Those who live in our most deprived areas and those with protected characteristics having more equitable access to services and improved experience of using services. More people who either have, or are at risk of, serious illness being diagnosed, supported and cared for at an earlier stage. More people being able to access services digitally more easily and feel supported to use technology in the best way to suit them and their needs.

Organisations and partners will be working differently together across four different levels to combine our collective energy, resource, expertise and the assets we already have within our communities.
