Overview An initiative to improve oral health and healthcare for older persons within the counties of Kent, Surrey and Sussex. The Oral Health Needs Assessment When should it be done? What is it? *On admission, as a matter of routine OR *As soon as this practice is adopted AND *At specified intervals to monitor changing needs * A risk assessment to identify problems and risk factors * Individual assessment of need * Facilitates appropriate response * Reflects good practice * Based on guidance * Contributes to planning and delivery of daily care Who should do it? *Any member of the team OR *Self assessment by able residents (2) Guidelines for Oral Health Care for Long-stay Patients and Residents. Daily Mouth Care Plans A care plan which: *Ensures appropriate care is provided *Records planning and delivery *Is specific to the individual concerned *Promotes a tailored daily routine *Demonstrates good practice *Provides daily documented evidence of care *Should be used by all members of the care team and reviewed regularly. 5
