Special Monthly Compensation (SMC) is a tax-free benefit for veterans, their spouses, surviving spouses, and parents. It provides higher rates due to special circumstances like the need for aid or specific disabilities. This additional compensation addresses various anatomical losses or impairments that are service-connected. The responsibility for determining loss of use of extremities lies with the rating activity, not the examining physician.

For Veterans, Special Monthly Compensation is a higher rate of compensation paid due to special circumstances such as the need of aid and attendance by another person or by specific disability, such as loss of use of one hand or leg. For spouses and surviving spouses, this benefit is commonly referred to as aid and attendance and is paid based on the need of aid and attendance by another person.

  4. Special Monthly Compensation (SMC) Special monthly compensation (SMC) is an additional level of compensation to Veterans (above the basic levels of compensation payable based on disability ratings of 0 to 100 percent) for various types of anatomical losses or levels of impairment due solely to service-connected (SC) disabilities. 4
