Organisation UHNM serve around three million people Around 1,450 inpatient beds across two sites in Stoke- on-Trent and Stafford 12,000 staff across both sites Specialist centre for Interventional Cardiology Major Trauma & Neurosurgery Acute Stroke & Neurological Centre >160,000 patients attend A&E yearly
What is TED? Coordinated approach to facilitate intensive therapy with a focus on functional activities to combine therapeutic interventions with supporting the patients care requirements (personal care, toileting, meal preparation) in the patient s own home
Rationale Avoid patients staying longer within the acute trust due to lack of social care in the community Avoid patients going to interim community beds while awaiting for a package of care Reduce deconditioning and risk of infection associated with prolonged hospital stay Previous patient experience demonstrated that people get better quicker in their own environment
Breaking Barriers Collective leadership from internal partners across the hospital divisions (specialised division & therapy department) Bring together teams across the system in both health and social care (acute & community trusts)
How we did it The TED Service was offered for 10 days and provided the intensity of up to 4 therapeutic interventions per day Review of their care requirement within the MDT in conjunction with the patients and their families All patients identified as medically fit for discharge home were triaged by the TED therapists The workforce was supported from the existing allied health professional workforce and therefore this allowed for patients to have access to an established and skilled workforce
Pilot 15 medical patients 5 specialised (4 T&O / 1 Neurology) Length of stay was reduced by 3.2 days Reduced reliance on social services on discharge from the acute hospital Reduction of care needed
Extended pilot T&O and neurology enhanced ESD 60 patients 6 months Reduced LOS by 6.7 days At the end of the extended pilot, 98% of the patient cohort had no social care package needs and were transferred under the community therapy service
Patient impact Positive impact on patient mood and motivation Less frustration from patients that were waiting for a package of care Regaining dignity and independence Patients feeling safer and less isolated
