  1. Integrated Health Committee DISCUSSION ABOUT MISSION, VISION, AND GOALS October 6, 2020

IH Committee 2020-21 Progress Mission Statement: LARRP s Integrated Health Committee is a dedicated space for behavioral health stakeholders working at the intersection of reentry, to convene regularly to promote harm reduction practices in reentry services and advance solutions to creating a continuum of care for people leaving incarceration and living with convictions. Committee goals include: Track and engage around statewide CAL-AIM process Advance the utilization of MAT among LA County reentry providers Identifying gaps in services and how to best collaborate/link Support the passage the SCS Bill (SK will champion locally) Advocate for the expansion of medication for opioid use disorder in LA County Jail COVID-19 and incarcerated population; increasing release rates and links to providers

Social Determinants of Health (SDoH) Circumstances in which people are born, grow, live, work and age, and the systems put in place to deal with illness Shaped by a wide set of forces, including economics, social policies, and politics Largely responsible for unfair and avoidable differences in health status CDC | WHO

IH Committee Vision HOUSING COMMITTEE EMPLOYMENT COMMITTEE EDUCATION COMMITTEE FAITH COMMITTEE INTEGRATED HEALTH COMMITTEE Specific Health Issues in the Re-Entry Population: - Chronic Medical Conditions: Diabetes, HTN, ESRD, Hep C - Serious Mental Illness: Depression, Bipolar Disorder, Anxiety Disorder, Schizophrenia - Substance Use Disorder - Preventative medicine: age appropriate cancer screening, immunizations/vaccine External Organizations to Partner with: - Department of Health Services (Whole Person Care/ODR/H4H programs) - Department of Mental Health - Department of Public Heath - Health Systems in LAC (should compile a list with contacts and divide up by region): UCLA, Cedars, Kaiser, St. Johns, California Hospital, MLK Community Hospital, FQHCs (AltaMed, Queenscare, Umma Clinic) - Community Based Organizations (should compile a list with contacts and divide up by region) that delivery health/mental health/substance use treatment **Consider regions of Los Angeles Countgy

IH Committee- Potential Mission & Goals Mission Statement: LARRP s Integrated Health Committee is a dedicated space for physical, mental, and behavioral health stakeholders working at the intersection of reentry, to convene regularly to promote holistic, harm reduction practices in reentry services and advance solutions to creating a continuum of care for people leaving incarceration and living with convictions. Goals: Advance utilization, education/training and policy related to Medication-Assisted Therapy Serve as a liaison between LARRP committees and external organizations across the Los Angeles County to address social determinants of health (e.g. housing, education, employment, etc.) of re-entry populations Lived experience project Collect, share and disseminate information/resources related to providing health, mental health and social services to re-entry populations
