If at anytime a student athletes insurance policy lapse, was not active at all or he/she will be removed from the team roster.
Intercollegiate Athletic Insurance Policy Bloomsburg offers intercollegiate athletic insurance, which is SECONDARY COVERAGE and is considered an EXCESS only policy Policy Coverage Injuries sustained in official team activities: games, practices, weight training, conditioning and travel. Non-Coverage Non-Athletic Related Injury: Illness, pre-existing conditions, etc. Any injury occurred in a non-sanctioned practice, game or conditioning ie. Intercollegiate Athletic Insurance Policy Cont. Student athletes are responsible for all primary insurance co-pays & deductibles prior to the Bloomsburg Athletics EXCESS insurance coming into affect. $2,000.00 deductible must be met by the student athlete s primary insurance or out of pocket before the secondary athletic policy will come into affect. It is the student athlete s responsibility to get all bills, EOB, etc. to the insurance advocate at Bloomsburg.
Proof of Insurance A copy of both front & back of the most recent & active insurance card MUST be on file with the athletic training department. Failure to show proof of insurance; the student athlete will NOT be permitted to participate in any athletic competition, practice, conditioning, etc. until resolved. If a new card is issued, it must be presented ASAP to athletic training staff Delays in medical treatment/imaging or other diagnostic testing can occur if updated cards are not on file
Helpful Tips for Proper Medical Coverage Know your primary medical coverage limitations, many company policies have limited coverage areas/network which will not permit medical services while at college. Changing primary care provider/physician to local physician in the Bloomsburg area. Provide insurance company with an updated student enrollment form. Supplemental insurance or add-ons maybe available to extend medical coverage into this area. ie.
