Lanark County Situation Table Lanark County Child & Youth Services Collaborative Oct. 10, 2024
Background A Community Safety Model In 2015 Pilot project brought multiple human- service sectors together to collectively identify systemic issues and risk factors prevalent locally and provide a network of support in order to prevent crisis situations. Two main components: 1. Situation Table with agencies collaborating to identify and intervene in situations of acutely elevated risk 2. Development of a community plan for safety and well-being that identifies local prevalent risks and strategies to mitigate them
Lanark County Situation Table Brings front-line agencies together (i.e. education, health/mental health, social services, housing, justice, addictions, victim services) Meet twice monthly or ad hoc to identify situations involving individuals of all ages who are at an acutely elevated risk of harm Intervene with appropriate, rapid support before crisis point; respectful of privacy uses a four-filter process supported by guidelines from IPC Not a case-management tool, nor is it a venue for self-referrals.

Lanark County Situation Table Highlights Approx. 27 direct service agencies involved and some ad hoc Dec. 9, 2015-Oct. 9, 2024: 364 discussions, 283 met AER threshold, 81.6% of those closed with overall risk lowered Top referrers: Police (45% combined); increase in FCS referrals over last two years Probation/Parole, LCMH/LLGAMH and Victim Services top lead/assisting agencies Mental health, criminal involvement and drugs top 3 risk categories (MH, criminal involvement and parenting in 2024) 55% of discussions involved youth ages 12-24 (62.5% so far in 2024)
Using the Lanark County Situation Table Filter One Filter Two Filter Three Filter Four Agency screening prior to introduction to the Situation Table De-identified discussion at Situation Table multiple risks, multi-agency response Determine which agencies should be involved Limited information shared in full in-camera discussion
Appropriate Referrals Demonstrate acute elevation Exhaust all options Are all multi-sectoral Come from member agency (including non-participating) Risk factors are pertinent to the current risk (limit to most important ones) Justifies information sharing Leave longer-term work for those who do case management Not just a place to dump hard-to-handle clients Risk = Something could happen Crisis = When it happens
Agency Processes Making a Referral Connect with your agency s representative at the situation table to determine if it is a good fit Complete a referral form Be available to take part in the discussion process as needed *If your agency is not a member agency of the LCST, contact the Coordinator directly
