LEXINGTON FIRE DEPARTMENT LEXINGTON FIRE DEPARTMENT Serves approximately 326,000 permanent residents over a 285 square mile area in Fayette County LFD also provides specialty resources for use throughout the State of Kentucky as the need arises Current operations: 23 Engine Companies 7 Ladder Companies 12 ALS Ambulances Community Paramedicine Pilot Program HAZMAT & Technical Rescue All are equipped with ALS equipment and can provide EMS Services Authorized personnel strength of 599 All are EMT-Basic 320 Paramedics (53%) Yearly EMS run volume is approximately 50,000 calls for service

A NEW APPROACH A NEW APPROACH LFD was approached by Eastern State Hospital (ESH) in the Spring of 2021 about transporting patients directly to ESH on a trial basis An MOU was signed between UK-ESH and LFUCG in the Winter of 2021 A subset of 125 patients from 2020 were identified to have suicidal ideations (SI) with no associated injuries These patients were chosen because they could have benefited the most from being taken to a higher level of mental healthcare sooner The goals of the program are: Ensure patients who do not require acute medical emergency services are placed in the most appropriate and cost effective level of care Reduce unnecessary hospital readmissions for patients Create an environment where all parties are able to create a hospital patient management system which improves patient outcomes

PILOT PROGRAM PILOT PROGRAM Fall 2021 ESH and LFD EMS worked together to develop a training program that was presented to ESH staff, LFD Providers and Lexington Police Minor injuries Simple contusions Abrasions Simple suturing NO Injuries that include: Fractures Ligature marks Ingestions Falls over 3-5 feet Education started in Winter 2021/2022 with a start date of March 2022 Behavioral Heath Transport Advisory Committee communicate on a regular basis to adjust the program as needed

PILOT PROGRAM OBSTACLES Misunderstanding of the program on the part of providers both in the hospital and EMS setting LFD change in our thinking Training cycles of the Lexington Police Department Unfamiliarity of the concept of patient destination choice Patient s familiarity of how the system works Stigma of being transported and treated at a mental hospital as opposed to a hospital

  6. DATA 1593 Total Calls for Service from March 2022-September 2022 1002 Transported to UK Behavioral Health (63%) 62 Transported to Eastern State Hospital (4%) 44 Transported to VA (3%) 182 Transported to other facilities (11%) 303 Not Transported (19%)
