SIM Overview n to test whether new payment and service delivery models will produce superior results when implemented in the context of a state-sponsored State Health Care Innovation Plan. These plans must improve health, improve health care, and lower costs for a state s citizens through a sustainable model of multi-payer payment and delivery reform, and must be dedicated to delivering the right care at the right time in the right setting. 3
Overarching High-Level View of Maine s SIM Grant n This grant will position Maine to assess the full impact associated with existing healthcare test reform models by moving them to the next level through: Enhanced care delivery capabilities Greater access to high-value care information and data Enhanced care delivery actor (provider and patient) training / support Introduction of targeted incentives 4

SIM Maine Leadership Team n 1. Maine Leadership Team (MLT) o Fundamental Accountability/Role: The State has overall accountability for the achievement of the Grant objectives; as such its role is to be the final decision-maker on issues impacting scope, cost and timelines and assurance that the Grant overall work remains aligned with the Grant overall objectives o Expectations of the State Executive Leadership Team: Accountable for fulfilling reporting requirements of CMMI Accountable for managing the Partner/Contractor relationships including contract management Accountable for financial Grant management Consistent understanding of status against plan overall Final arbiter on escalated issues impacting scope, cost or timeline and on issues the Steering Committee cannot reach consensus agreement on Designate the Steering Committee Chair Assure that MaineCare-specific objectives are managed and achieved through the Grant Manage periodic stakeholder meetings The MLT will be responsible for approval of changes to budget and scope of work, and any major shifts in State resource allocation and decisions requiring senior State authority. The SIM Program Manager will make monthly reports to the MLT on performance, financial, workplan progress, and any recommendations coming out of the Steering Committee. 8
Operational Plan Development and Review: n Timeline and Process July 10: Steering Committee Begins Review and submits requests for more information Aug 1: June 10 28 Operational Plan Development Submission of SIM Operational Plan to CMMI June 28: Deadline for Content Submission July 27: Steering Committee Approval for Submission Oct 1: Deadline for CMMI approval to move into implementation phase 9
