The ELFT System Merger of Victoria Medical Centre and Five Elms Medical Practice 5-8 July 2024 elft.nhs.uk
What is happening? Over the weekend the Five Elms Medical Practice s computer system will be merged into the Victoria Medical Centre database. This will be conducted by the supplier (EMIS) supported by the local IT team. This includes a system back up at both sites and list reconciliation on 8 July 2024. There will be no access to Five Elms Medical Practice database after this date. On 8 July 2024, when the system goes live, there will be an EMIS engineer and support staff at the sites to assist. elft.nhs.uk
Why is this happening? With the two practices merging, we will be able to provide patients with more services from a wider range of experienced clinicians. The merger will also provide security for both practices, enabling primary care to thrive and become more sustainable and resilient providing high quality of care for all patients. elft.nhs.uk
Timeline and Key Points Five Elms Medical Practice remains a business as usual prior to the merge date. The teams at this site will not have any access to the records of Victoria Medical Practice s patients until after the go live on 8 July 2024. elft.nhs.uk
Timeline and Key Points 7 Days Prior the Merger at Five Elms Site No new patient registrations will be processed; registrations will be only accepted at Victoria Medical Centre. Online services will be withdrawn we appreciate the inconvenience this will cause. Test results will be sent via email instead of directly to the EMIS system. This may cause a slight delay in updating patient records. Urgent results are phoned directly to the surgery as a failsafe measure. Future appointment bookings will be temporarily restricted to prevent duplication. elft.nhs.uk
Timeline and Key Points Friday 5 July 2024 at Five Elms Site There will be no access to the database until 8 July 2024. NHS 111 will provide cover during the afternoon and patients will be able to contact NHS 111 for any urgent medical treatment. Please contact 999 for medical emergencies. Timeline and Key Points Weekend (5-8 July 2024) Databases will be merged by EMIS. This is an external process. Out of hours cover will be available in the usual way; please note that EMIS may not have access to your GP medical record. elft.nhs.uk
Timeline and Key Points Monday 8 July 2024 Merged database opens at both sites. Most appointments on this day will be available for booking on the same day for urgent matters. This allows flexibility for any unforeseen issues. Appointment booking will resume as normal on 9 July 2024. elft.nhs.uk

Starting from 24 June, when you request your medication, please make a note on your request if you would like this to happen. All clinical and non-clinical staff have smartcards which define their level of access. Access can be audited if you have any specific concerns; these can be raised with the practice manager.
