  2. LTFU Focus Call Welcome and introductions Name Role at center and VQI Charter? Status There were 25 participants on the call!! A few first timers. Reminded everyone we encourage participation. This is a judgement free zone. Perfect setting to ask questions and share. 2

  3. Discussion Round table: Where are you in the process? Thinking about it, starting, mid-way, wrapping up, sustaining Barriers Problems encountered when other than vascular providers discharge the patient (Interventionalists). Suggestions included sending letters to the patient s primary. Sample letters are on the website: https://www.vqi.org/resources/discharge-medication-communications/ You ll need a different log on and password for this. Click here for sample charters: https://www.vqi.org/national-data/ Scroll to the bottom of the page and click on 2 . In the middle column at the bottom, you ll see links to different sample charters. You re free to use any portion of any of them, just plug your data into it. Send the charter to me and I ll review and give you pointers. Also, you get 2 QI points towards the participation awards. Successes Sending letters to primary physicians, educating other specialty providers Discussion brought up about the confusing help text with Pre and Post Procedure Niacin: Response: Per the Medical and Associate Medical Directors, and in alignment with the Post Statin Help Text, stand alone niacin is not considered statin therapy. Combination preparations of medications, as seen below, are considered statin therapy. The pre and post statin help texts will be aligned with the next quarterly update the match the help text above. Help Text = Any of the HMG-CoA reductase inhibitors used to reduce cholesterol, including atorvastatin (Lipitor and Torvast), fluvastatin (Lescol), lovastatin (Mevacor, Altocor, Altoprev), pitavastatin (Livalo, Pitava), pravastatin (Pravachol, Selektine, Lipostat), rosuvastatin (Crestor), simvastatin (Zocor, Lipex); ezetimibe (Zetia); Niacin (Vitamin B3) or combination preparations of a statin and another agent - such as ezetimibe/simvastatin (Vytorin); lovastatin/niacin SR (Advicor); ezetimibe/atorvastatin (Liptruzet); amlodipine/atorvastatin(Caduet); sitaglitin/simvastatin (Juvisync); niacin/simvastatin (Simcor) 3
