Introduction & Background BCBSMN has contracted with Open Arms of MN to provide condition-specific, home-delivered meals and nutrition education to your SecureBlue (MSHO) members. This presentation will provide you with details about the program and guide you through the steps of making a referral.
Open Arms of MN Mission: To nourish body, mind and soul. Purpose: To build a joyful and nourishing community that provides medically tailored meals and hope for people experiencing a significant health crisis or living with a life-threatening illness. Founded in 1986 in response to HIV/AIDS, expanded to other illnesses including cancer, CHF, ESRD, ALS, MS, and COPD. Serve 1000+ clients per week Community of 7,500 active volunteers 620,000+ meals delivered in 2018 5 urban farms, summer meals program
Blue Plus Medically Tailored Meals Benefit Chronic Condition Medically Tailored Meal Program (Open Arms) MSHO members diagnosed with or at risk for one or more chronic conditions are eligible to receive two meals per day for a period of twelve weeks, and up to 150 minutes of nutrition education. Post-Discharge Medically Tailored Meal Program (Mom s Meals) MSHO members discharged to home from an inpatient or short-term nursing home stay are eligible to receive two meals per day for a period of four weeks, and up to 120 minutes of nutrition education. Post-Discharge + Chronic Condition (Both) MSHO members discharged from an inpatient or short-term nursing home stay and are also diagnosed with or at risk from one or more chronic conditions are eligible to receive two meals per day for a period of sixteen weeks, plus up to 150 minutes of nutrition education. Note: Open Arms conducts outreach for both benefits.
Blue Plus Medically Tailored Meals Benefit Open Arms will deliver meals to ALL members who participate in the Chronic Condition program. Mom s Meals will deliver meals to ALL members who participate in the Post-Discharge program. If a member participates in both programs, they will receive Mom s Meals for 4 weeks following discharge and Open Arms meals for the remaining 12 weeks. Open Arms will provide nutrition education to ALL participating members.
Blue Plus Medically Tailored Meals Benefit - Eligibility To qualify for the Chronic Condition Medically Tailored Meals & Nutrition Education Program, MSHO members must be diagnosed with or at risk for chronic conditions known to benefit from nutrition interventions, including the following: Cancer Chronic Obstructive Pulmonary Disease (COPD) Congestive Heart Failure (CHF) Coronary Artery Disease (CAD) Diabetes End-stage Renal Disease (ESRD) HIV/AIDS Peripheral Vascular Disease Rheumatoid Arthritis Stroke Other: If your member does not have one of these qualifying conditions but would benefit from this program, discuss with your Partner Relations Consultant (PRC) to obtain approval. Open Arms will target outreach to those with COPD, CHF, and Diabetes. These members may be contacted directly by Open Arms to enroll. You will be notified before and after this contact.
Blue Plus Medically Tailored Meals Benefit - Eligibility To participate in the Chronic Condition Medically Tailored Meals & Nutrition Education Program, the following is required for eligibility: Members can participate if they are currently receiving 0 or 1 meals per day from another source, such as their living facility or another home delivered meals provider. If members receive 2 or more meals per day, they are not eligible for this benefit. Members must participate in nutrition education to continue the meal deliveries: At least one follow-up call per month, up to 150 minutes total. Open Arms will attempt to reach clients for scheduled appointments 3 times, if they cannot reach the member on the 3rd try, the meals may be suspended until communication has been made. You will be notified PRIOR to any meal suspension
Blue Plus Medically Tailored Meals Benefit - Eligibility To qualify for the Post-Discharge Medically Tailored Meals & Nutrition Education Program, MSHO member must have been discharged from an inpatient or short-term nursing home stay within seven business days of the Care Coordinator submitting the referral form. Open Arms receives a daily discharge report from Blue Plus and may contact you to help get these members enrolled in a timely manner.
Medically Tailored Meals Blue Plus & OAM Addtl Details Member will receive a welcome call from an OAM nutrition specialist, which includes the following: Initial nutrition assessment Offering of continued nutrition coaching sessions with a licensed dietitian to answer questions related to eating habits and overall health Set up meal delivery Meals from Open Arms are delivered directly to members homes Members living in the metro area will generally receive their meals between 11:00am and 2:00pm on they day they choose (M-F) during their welcome call. Someone in the household must be home to receive the meal delivery. Members living in Greater MN will have their meals shipped to them via UPS (meals will generally arrive on Wednesdays or Thursdays) Note: Meals are packaged to be safe up to 48 hours from time of packaging.
Medically Tailored Meals Blue Plus & OAM Addtl Details Members will receive their first meal delivery within 1 week of completing the welcome call. All 14 meals will be delivered on a weekly basis to the member s home. Every weekly delivery includes nutrition information and recipe ideas. At program end members will receive a free cookbook.
How to Make a Referral 1. Locate the OAM Referral Form on the SecureBlue MSHO Supplemental Benefits page of the Blue Plus Care Coordinator Portal under Medically-Tailored Meals & Nutrition Education 2. Complete the Form 1. Indicate the member s benefit (chronic condition or post-discharge*) 2. Note any dietary preferences or allergies and any delivery details 3. Obtain Partner Relations approval for any Other diagnosis 4. Contact OAM with questions by calling 612-677-2576 or emailing referrals@openarmsmn.org 3. Send Form to OAM via secure email or fax 1. Why Should MSHO Members Participate? The Medically Tailored Meals & Nutrition Education Program is designed to support MSHO members in making sustained dietary changes to better manage their health. Members will receive meals tailored to help manage their diagnoses. Menu options include low-sodium, heart healthy, diabetes-friendly, and dialysis-friendly. Nutrition education will be provided by a licensed dietitian and will include coaching on how to shop for and cook foods that meet the member s dietary needs. Receiving meals along with nutrition education will help the member to understand appropriate portion sizes and healthy ingredient choices.
What are Medically Tailored Meals? MTM: meals approved by a registered dietitian that reflect appropriate dietary therapy to address medical diagnoses, symptoms, allergies, the inability to chew or swallow, medication management, and side effects to ensure the best possible nutrition-related health outcomes. MTM are often paired with medical nutrition therapy or education by an RD. Together, we believe this provides the best health outcomes and disease management. Medically Tailored Meals at Open Arms Clients choose one of 5 menus, and receive weekly deliveries Heart Healthy Standard diet option; includes a variety of foods that support heart health and general nutrition needs. Kidney-Friendly Diet for clients on hemodialysis; limits potassium, phosphorus, sodium, and high in protein. Bland Diet low in spice/acid for taste changes. Fork mashable texture for chewing/swallowing difficulties. Vegetarian No meat, poultry, or fish. May contain dairy/eggs. Pureed (any menu above) Current clients get 14 meals per week or 50-60% of RDA for calories, protein, carbs, and fat Recipes written by chefs and approved by RDs 95% meals made from scratch, including fresh fruits and vegetables All menus are low in salt and saturated fats, as well as diabetic friendly
Communication from Open Arms We may be in contact with you with questions as orders are processed, to help with getting members enrolled in a timely manner, and to update you on members care plans. You may receive secure emails from Open Arms, some that require immediate response. Contact us ASAP if you have questions or trouble opening the emails.
