Introduction Introduction The purpose of this report is to update the Trust Board on the following:- The results of a GAP analysis undertaken on the Trust Health and safety management systems and approach The results of an internal audit report The actions taken and proposed to develop and improve the Trusts Safety management Systems and approach.

  4. GAP Analysis and Internal Audit GAP Analysis and Internal Audit The Trust s inhouse Health and Safety team have undertaken a GAP analysis of the Trust current health and safety systems. Although the number of incidents and reportable accidents is not exceptional for the number of staff employed, the review identified key areas of improvement needed and an action plan to deliver these improvements has been developed. A recent internal audit of the H&S processes has also been undertaken by PWC. This identified a number of concerns. The majority of the concerns noted had already been identified by our internal GAP analysis. The 3 concerns noted below had not been identified previously. o Development of role specific risk assessments o Transfer of policies to the new template o RIDDOR reporting issues The concerns raised regarding RIDDOR reporting have been resolved by the Quality and Risk Team, and the concerns re the role specific risk assessments and policy templates have been included in our action plan. A further review of processes has recently being concluded by an external consultant currently employed to support the development of the health and safety function. This review identified one concern not identified by our internal review the management of the Trusts grey fleet (ie. Personal cars used for Trust purposes).

  5. Proposals to improve and develop Proposals to improve and develop An Action Plan detailing areas of concern and improvement actions is in place and this is monitored and updated. The action plan identifies two key issues which must urgently be addressed:- Resourcing Training A paper detailing our proposals to address these key issues has been drafted for consideration.

Summary of Health and Safety Incidents Summary of Health and Safety Incidents The following pages provides a summary of the Health and Safety incidents relating to the categorise listed below that have occurred throughout the Trust during the first 5 months of this year. Violence and aggression Falls Manual handling Collisions .

  7. Violence and Aggression Violence and Aggression Overview Overview Violence and aggression against staff is a serious concern. In the first 5 months of this financial year a total of 392 incidents have been recorded. Four violence and aggression incidents have resulted in the submission of a RIDDOR. Of these three related to Phoenix Ward. The majority of these incidents have been rated by the reporting manager as incidents resulting in no-harm and there is concern that this is reflective of a culture where violence and aggression is expected and tolerated as part of the job. Whilst the majority of violence and aggression incidents relate to our mental health services, the recent months have seen an increase in those reported by Community Services. The extent, impact and management of violence and aggression is a concern and has been identified by the Health and Safety team and the Health and Safety Committee as an area requiring more detailed understanding and the development of a plan to reduce the current levels. This will build on the excellent work done to date by the Zero Tolerance Group.

  9. Falls Falls Overview Overview In the first 5 months of this financial year a total of 22 Falls incidents have been recorded. The majority of these incidents have been rated by the reporting manager as incidents resulting in minor harm. The majority of staff falls (41%) have occurred within the Community Services . No trends (ie. areas/types of incidents) have yet been identified.

  11. Collisions Incidents Collisions Incidents Overview Overview In the first 5 months of this financial year a total of 17 Collision incidents have been recorded. The majority of these incidents have been rated by the reporting manager has incidents resulting in minor harm. However two were rated as severe. Both these incidents involve vehicles. They were not RIDDOR reportable. The majority of collision incidents (83%) have occurred within the Community Services . No trends (ie. areas/types of incidents) have been identified.

  13. Manual Handling Incidents Manual Handling Incidents Overview Overview In the first 5 months of this financial year a total of 16 Manual handling incidents have been recorded. The majority of these incidents have been rated by the reporting manager has incidents resulting in minor harm. However one resulted in the submission of a RIDDOR (over 7 days absence as a result of the incident) The majority of collision incidents (50%) have occurred within the Community Services . No trends (ie. areas/types of incidents) have been identified.
