Introduction The level to which a patient is sensitised against the general donor population can play an important role in organ allocation: - 2006 Kidney Allocation Scheme Highly sensitised patients that are zero HLA mismatched receive priority over patients with lower levels or no sensitisation Software is available in the UK that allows the transplant community to calculate a standardised sensitisation percentage

Database - donors Five donor files received: - Barcelona - Eurotransplant - Greece - UK - Prague: no Cw typing - excluded Files contain: - Donor HLA type A, B, DR, Cw, DQ - Donor blood group Some partners have only recently collected HLA-Cw & DQ - relatively small donor populations

Results - Eurotransplant Number of HSP long waiting patients: 121 Patients that benefit from different donor pool: 27 (22%) - 19 patients 1% increase - 5 patients 2% increase - 2 patients 4% increase Barcelona donor pool - 1 patient 5% increase UK donor pool
Results - Greece Number of HSP long waiting patients: 113 Patients that benefit from different donor pool: 45 (40%) - 25 patients 1% increase - 7 patients 2% increase - 4 patients 3% increase - 3 patients 4% increase - 3 patients 5% increase - 1 patient 7% increase Barcelona donor pool - 1 patient 10% increase UK donor pool - 1 patient 23% increase UK donor pool
Results - UK Number of HSP long waiting patients: 334 Patients that benefit from different donor pool: 89 (27%) - 58 patients 1% increase - 10 patients 2% increase - 11 patients 3% increase - 4 patients 4% increase - 3 patients 5% increase Barcelona donor pool - 3 patients 9% increase Greece donor pool
Results - Prague Number of HSP long waiting patients: 23 Patients that benefit from different donor pool: 4 (17%) - 1 patient 2% increase - 1 patient 5% increase - 1 patient 9% increase Greece donor pool - 1 patient 27% increase Barcelona donor pool
Summary Already established UK tool to estimate proportion of UK HLA compatible donors Tool can be applied to different European donor populations - Demonstrated potential for very difficult to transplant patients Using these simulations a pilot could be conducted as a proof of principle
