  2. Aims and Measures Our aim was to Introduce Transitional Care within the Neonatal Ward and Maternity In-Patient Ward to all relevant families by March 2019 Measures Reduction in Term Admissions to the NNU Family Satisfaction

PSDA Ramp Change 4: Use of four bedded area in In-patient ward A P S D Change 3: Use of one family room in the In-Patient Ward Change 2: Altered the use of family room NNU A P S D Change 1: One parent in family room NNU Improvement methodology also used utilising PDSA in the identification and training of appropriate staff

Testing, Implementing and Spreading Testing The Use of appropriate areas Staff education Implementing Use of one family room in NNU Designated cohort of staff in In-Patient ward trained Guideline and training pack tested and implemented Spreading The use of family rooms in NNU The use of four bedded area in In-patient ward All MCA staff trained in In-patient ward Designated cohort of midwifery staff trained in In-Patient ward

Teamwork & Engagement Identified Team Leads (NNU & In-Patient Ward, AMU) Identified Neonatal Buddy to work with MCAs in In-Patient Ward One to One Training provided for staff who required additional mentoring Promotion and sharing of work discussed at department safety briefs and AMU safety brief Poster to allow staff to provide feedback visible in both departments

Successes and Challenges Success Empowering parents Avoiding unnecessary separation of mothers and babies Increasing NNU capacity and allowing earlier repatriation Increased opportunity for parental peer support Commitment from the staff to make it work Staff development (maternity care assistants developing skills) Enhancing communication and collaborative work between Neonatal and Maternity Services Challenges Identifying the correct environment (space) Identifying the appropriate management structure Staff engagement
