  3. Why Do People with Dementia Fall? Physical weakness, gait changes poor balance Difficulties dual tasking Type of dementia Stress and Distress Visual misperception Memory impairment and disorientation Impaired judgment Medication side effects Depression Orthostatic hypotension

  4. Project Aim The aim of this project is to reduce the number of falls in a participating care home by 50% by end December 2016. The aim was based on previous intelligence from Up and About in Care Homes Project

  7. Change Ideas Process Mapping Workshop mapping of resident journey. Potential areas for improvement were flagged in red Potential areas for improvement were discussed, themed into 3 main areas and prioritised by staff staff knowledge/engagement, Multifactorial risk assessment, post fall analysis 1st PDSA -80% care staff to be trained by end July, 95% compliance with x2 daily staff handovers by 12th June 2nd PDSA - 90% of residents will have an up to date multifactorial risk assessment by the end of October 2016

My Improvement Journey Highlights my learning, spending time with care home team, use of control charts, the results Challenges not working in the area the improvements are happening, having no managerial authority, the refurbishments, staff changes, maintaining momentum Tips use PDSA - have a theory, make a prediction, measure to check progress, do not go on your journey alone, start smaller then you think you should, don t give up adapt, adopt, abandon
