Older Adult Isolation Imperative 19.6% of Lackawanna County population is over 65, versus 17.7% in PA and 15.6% in the US overall To reach older adults at high risk for negative health outcomes prioritize social isolation, which has direct impacts on: Blood pressure Chronic disease Depression Vulnerability to abuse and neglect

Focusing Our Approach An array of direct services for isolated older adults already exists in our community, but we lack tools to identify, reach, and connect isolated individuals to these services.   6. Highlights of RFP Approach Collaboration Why collaborative funding? Agencies expressed desire for additional collaboration Complex nature of isolation Capacity to achieve ultimate goal: By the end of 2023, we will ensure Lackawanna County health and social service providers serving older adults are using a tool to measure social isolation and connecting interested clients to navigators. Community navigators will provide direct service to reduce social isolation in 3,000 older adults isolated or at risk for isolation.
