How to Submit Your Research-Based Certification UAMS Responsibilities Page: answer ALL seven questions and click Continue. If you have previously submitted an annual certification, the questions will already be answered. Please review your answers, change answers as needed and click Continue. 7/12/2025 5 7/12/2025
How to Submit Your Research-Based Certification What to Disclose Page: This provides information on what outside interests must be disclosed. Answer the four questions at the bottom of the page and click Continue. If you have previously disclosed in this system, please review your answers, change answers as needed and click Continue. 7/12/2025 6 7/12/2025
How to Submit Your Research-Based Certification Disclosure Details Page: Click on Add Disclosure to add information about an outside interest. (Move through the next five slides for detailed instructions on adding a disclosure. Advance to slide 13 if you do not need instructions on adding disclosures.) How to Submit Your Research-Based Certification Select the Organization from the list by typing the first few letters of the name. If you are unable to find your organization, enter the name in the second text box. Click OK. 7/12/2025 8 7/12/2025
How to Submit Your Research-Based Certification General Information Page: Indicate if you or an Immediate Family Member has the outside interest and select the types of interests you have with the organization. Click Continue. 7/12/2025 9 7/12/2025
How to Submit Your Research-Based Certification Specific Information about outside interest: Answer all the questions related to the type of interest you selected and click Continue. If you selected more than one type of interest for an organization, you will have a page with questions for each type of interest.
