YOUR GROUP Action plan implemented over 2 years: Year 1 Establish all 3 tiers in Inverurie and Kemnay networks Year 2 Continue in Inverurie and Kemnay networks and expand delivery to include Huntly network
YOUR SPACE Diversionary activities delivered within community Activities to promote physical and mental wellbeing, including issue based topics as required Police tactical analysis YP have ownership Referral pathways in and out targeted approach (GIRFEC), sustainability
GOOD LIFE CHOICES (GLC) Student-led 12 week group work programme to increase resilience, confidence and self-esteem through a variety of health and wellbeing activities Referral pathways in and out targeted approach (GIRFEC), sustainability (links to YOUR Space) Peer education approach Family member involvement
STRUCTURED 1:1 SUPPORT Strength based needs assessment Holistic action plan to reduce risks and harms, promote positive choices about health, wellbeing and future aspirations Regular reviews Partnership working (GIRFEC Named Person/Lead Professional) Referral pathways in and out tier 1 & 2, other resources, services and supports
AGREED OUTCOMES 1. Included: YP who are socially isolated (and their family members where appropriate) have improved access to appropriate support within the local community 2. Active: YP have increased opportunities to take part in a range of activities which contribute to healthy growth and development 3. Healthy: YP (and their family members where appropriate) have improved their ability to make positive choices about their health and wellbeing 4.
