Safety issues EO supply TODO ao Description Cause Correctiveaction Liquid and/or gas phase: seal mechanically damaged *Misalignment of TODO resulting in the application of excessive force *Unbalanced unloading arm/hose set-up *Rough Handling *Replace seal *Properly balance any unloading arm/hose *Rehearsal training operators Liquid and/or gas phase: seal chemically damaged Wear and tear due to not adhering to replacement frequency OR ersatz variety of Chemraz 505 *Replace seal *Verify seal certificate TODO-connection difficult to make due to backpressure between TODO/ball valve *Improper flushing of unloading line prior to closing the valves *Leaking bottom AND ball valve *Adequacy procedure to be verified /rehearsed *Overhaul of bottom/ball valve Gas /Liquid phase: dustcap available but not mounted ->potential for dirt/sand. Ingress. Not adhering to procedures at loading Terminal (driver/ operator) Use dustcap or dummy set-up Liquid phase leaking ~2/3 drops per minute *Positioning of seal OK? *Misalignment of TODO due to excessive force *Reposition seal *Properly balance any unloading arm/hose *Rehearsal training operators Liquid and/or gas phase: unlevelled face of TODO resulting in a leak >10 drops per minute *Improper maintenance during overhaul *Replace seal *Review maintenance procedure
