  2. The following slides provide sample wording to address each of the disclosure elements. Nurse Planners should determine which disclosures are applicable and ensure that each one is delivered to learners prior to the start of the activity, or initiation of educational content. It is strongly recommended that Approved Providers include all disclosures in one place, even if some appear again elsewhere such as on marketing materials.

  3. Nurse Planners must decide the most effective method to ensure that participants both receive and read the disclosure information BEFORE the start of the program. This can be accomplished by a variety of mechanisms, for example, the use of a slide, a slide loop , a handout, a Disclosures tab in a virtual platform, as part of the marketing materials for all delivery formats, etc.

PROVIDER APPROVAL STATEMENT DISCLOSURE [Insert Name of Approved Provider Unit] is approved as a provider of nursing continuing professional development by the Midwest Multistate Division, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation. The provider approval statement above cannot be added to, altered or changed in any way and must read verbatim. Do not add any additional numbers, event ID, or Provider Approval Number to statement

  7. Criteria for Awarding Contact Hours

CRITERIA FOR AWARDING CONTACT HOURS DISCLOSURE WORDING CrerCcCriteria for awarding contact hours disclosures are determined through the planning process and identified on the NCPD Activity Planning Guide. All completion requirements must be disclosed to learners prior to the start of the activity. Sample Wording: Live, in-person In order to obtain nursing contact hours, you must attend the entire activity, participate in case study analysis, and complete the evaluation. Live, in-person (breakouts/concurrent sessions) In order to obtain nursing contact hours, you must attend [insert number] breakout sessions in their entirety, submit a verification of attendance form noting the sessions attended, and complete the evaluation. Credit will only be awarded for sessions attended/completed. Live, virtual In order to obtain nursing contact hours, you must participate in the entire program, participate in audience polling and/or Q&A and complete the evaluation. Web-Based/On-Demand In order to obtain nursing contact hours, you must view the entire program, pass the program posttest with a score of 90% or better and complete the evaluation. You may take the posttest up to three times to achieve the passing score if necessary.

  10. Relevant Financial Relationship Disclosure

RELEVANT FINANCIAL RELATIONSHIP DISCLOSURE WORDING If the Nurse Planner determines that an individual in a position to control content for an activity with clinical subject matter does have a relevant financial relationship with an ineligible company/organization (relevant financial relationship is present), the activity providers must inform learners of the nature of the relationship and a statement of mitigation. Dr. Smith serves as a consultant for Pills, Inc. Pharmaceuticals. All relevant financial relationships for this individual have been mitigated. No other relevant financial relationships were identified for any member of the planning committee or presenter/author.

  12. RELEVANT FINANCIAL RELATIONSHIP DISCLOSURE WORDING If the Nurse Planner determines that the individuals in a position to control content for an activity with clinical subject matter do not have a relevant financial relationship with an ineligible company/ organization, the activity provider must inform learners that no relevant financial relationships exist. No relevant financial relationships were identified for any member of the planning committee or any presenter/author of the program content.

RELEVANT FINANCIAL RELATIONSHIP DISCLOSURE WORDING If the Nurse Planner determines that the individuals in a position to control content for an activity with no clinical subject matter do not have a relevant financial relationship with an ineligible company/ organization, no disclosure about financial relationships is required.

  14. Commercial Support

  15. COMMERCIAL SUPPORT DISCLOSURE The name and nature of commercial support from ineligible companies/ organizations is required to be disclosed to learners prior to the activity, when received. If the activity did not include commercial support, nothing is required to be disclosed. Do not include corporate logos, trade names, or product group messages Sample Wording: Ortho Plus DME has generously donated funds to support this program. Ortho Plus had no input into or influence over the content or choice of presenters. General Mills Library has generously allowed us to use their conference room space for this program.

  16. Joint Providers

  17. JOINT PROVIDER DISCLOSURE Joint Providers are required to be disclosed to learners prior to the activity. If the activity was not jointly provided, nothing is required to be disclosed. Joint Providers must also be disclosed on all marketing materials. Sample Wording: This seminar is jointly provided by Jones Bones Clinic and the Midwest Ortho Nurses Association.

  18. Expiration Date for Awarding Contact Hours for Enduring Materials

DISCLOSURE OF THE EXPIRATION DATE FOR AWARDING CONTACT HOURS FOR ENDURING PROGRAMS The expiration date for the awarding of contact hours for enduring programs must be disclosed to learners prior to the activity. If the activity was not an enduring activity, no expiration date is required to be disclosed. The contact hours themselves do not expire, but the ability to award those contact hours does have an expiration date. Sample Wording: Contact hours for this program will not be awarded after June 30, 2024
