  2. 2 2 Verification Process October 1 February 1 Count the number of approved applications. October 31 Deadline to submit the Verification Report (FNS- 742) to TDA via TX-UNPS. Determine the total number of students enrolled and sites for the current school year. Count student eligibility. Verification sample size: number of approved applications + the number of applications verified for cause. Submit Preliminary Verification Report to TDA. Matching student names to DV list (optional but encouraged) Verification Letters to households ARM, Section 6, Verification of Eligibility

  4. 4 Section 1: All CEs must complete Section 1, including Provision 2 (Base Year & Non-Base Year) and CEP CEs.

  5. 5 Section 2: CEs operating Special Provision Options only must complete Section 2. Operating Special Provision Options means CEs operating Provision 2 (Base Year & Non-Base Year) and CEP at any or all sites.

  6. 6 Section 3: All CEs must complete Section 3 or check box if not applicable. * Provision 2 Non-Base Year and CEP: Check the box at 3-1and enter 0 in 3-2, 3-3, and 3-4. * Provision 2 CEs in the Base Year: Do NOT check the box at 3-1 and do record the number of free students in 3-2, 3-3 and 3-4. CEs operating a summer program and did not collect any applications should enter 0 in Column B.

  7. 7 Section 4: All CEs must complete Section 4, including Provision 2 Non-Base Year & Base Year and CEP. * Provision 2 in a Non-Base Year and/or CEP: Enter 0 in 4-1, 4-2, and 4-3. * Standard Counting & Claiming and/or Provision 2 in a Base Year: Enter the number of applications and students for each category in 4-1, 4-2 and 4-3. CEs operating a summer program and did not collect any applications should enter 0 in Columns A and B.

  8. 8 Section 5: All CEs must complete Section 5. * Standard Counting & Claiming and/or Provision 2 Base Year: Complete 5-2, 5-3, 5-4, 5-5 & 5-7. Check the box on 5-6 if direct verification not utilized. If applications were not received and verification was not conducted, enter 0 in 5-4, 5-5 and 5-7. * Provision 2 Non-Base Year and/or CEP CE-Wide: Check the box at 5-1, and 5-6.These CEs enter 0 in 5-4, 5-5 and 5-7. CEs operating a summer program and did not collect any applications or conduct verification should enter 0.

  9. 9 Section 5-8: All CEs must complete Section 5-8. * Standard Counting & Claiming and/or Provision 2 Base Year: Complete 5-8 and VC-1 based on your verification efforts. If applications were not received and verification was not conducted, enter 0 in the boxes for 5-8 and VC-1. * Provision 2 Non-Base Year and/or CEP: Check the box at 5-1 and 5-6. These CEs enter 0 in the boxes for 5- 8 and VC-1. CEs operating a summer program and did not collect any applications or conduct verification should enter 0.
