MiSiS Out-of-Home Placement Updates MiSiS contains students from DataMatch list with an open DCFS case & an open court case. MiSiS is missing students from DataMatch list whose court case has been closed, but DCFS case remains open. MiSiS will soon contain students from DataMatch list who fall in both categories.

SOS Logs Updates Comprehensive Academic Assessment Column - Drop down menu with Yes and In progress - Use Yes when reasonable attempts (min 2) have been made to contact caregiver/parent and CSW. - Use In progress when pending information from MiSiS, MyData, Cumulative Record, and less than 2 attempts have been made to contact caregiver/parent and CSW.

Intensive SOS Logs Updates Summary of Service: Guide of Terms and Definitions Caregiver/ Guardian Contact Conference or contact made with caregiver, biological parent, relative, legal guardian, etc. (In person, phone or e-mail) School Site/District Personnel Contact Contact made with school counselor, deans, teachers, District personnel, etc. (In person, phone or e-mail) Non-LAUSD Support Staff Contact Contact made with DCFS, Attorney, Wraparound, Probation etc. (In person, phone or e-mail) Individual Interventions Referrals Students/ families referred to school or community resources (ILP, tutoring, or other resource/program, etc.) Monitoring, Reviewing and or Transferring Records Review of school records, MyData, MiSiS, MAT, Health records, etc. This also includes compiling and transferring of records between schools, districts, FYAP counselors, etc. Target Individual Student Contact Academic counseling, social-emotional counseling, attendance counseling, transition planning, etc. Multi-Disciplinary Meetings COST, SST, IEP, LAT, MAT, Wraparound,etc. (Facilitated by school site support staff or outside agency) Home Visits Home visit to caregiver, biological parent, group home, etc. Crisis Intervention/Threat Assessment Crisis intervention such as child abuse report, threat assessment (RARD), school refusal, etc. Group Interventions Student Groups Providing group intervention to students regarding academics, social-emotional, attendance improvement, etc. Universal Caregiver Engagement Group Meeting Workshops, support group for caregivers/parents. Collaborative Team Meetings FYAP facilitated meetings with school site support staff to discuss multiple cases. Preventions Mass Communication Events Parent Engagement Letter, Mass Communication Announcement, Assemblies Informational Presentation FYAP facilitated presentations on topics specific to foster youth to faculty, District personnel, community agencies, etc. Recognitions Events Participation in a recognition ceremony/event, or giving out incentives, donations, awards, etc.
