  1. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017)

  2. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER1, 2017) SCDDSN requires the use of the Therap General Event Reporting Module (GER) for all events listed in this document. The GER may be completed by the Direct Support Staff, or by individuals designed by the Provider Agency to do so. Notification timelines and additional reporting requirements are included in this document. Some GER Reports will also result in the need to complete a separate report on the DDSN Incident Management System. The GER will replace other documentation sources that a provider may have had in place to document such events. The GER should not become "another" place for staff to document an event. It should be "the" primary record/documentation.

  3. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) In general, all documentation must be entered by the end of the shift. The Event Date is the date the incident happened. The information in GER will not be required to be uploaded in IMS, but the user must reference the Therap Form ID. The GER will not be another place to document. It is the place for DDSN Contracted Provider staff to document required events. A provider s use of additional reporting logs or paper documents is optional, according to the provider s internal policy. The Provider staff will select the Event Type within the GER and then follow the prompts for additional drop-down selections.

SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Some event types will require a Critical Incident Report or Initial Report of Abuse, Neglect and Exploitation. Additional reporting requirements using the DDSN Incident Management System are noted within the GER matrix. The provider agency designee must review and approve the GER according to their internal policies. The designated staff may enter additional comments for GER Follow-up and determine routing. For example, the designee may require notification and review by the agency nurse for any injuries. GERs will appear on the dashboard for Case Managers for anyone assigned to their caseload.

SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Category DDSN Event Type THERAP GER Event Type/ Sub-type Notification Timeline Means of Discovery Definition Many Medical Events previously required as Critical Incidents will now be entered in Therap as a GER. Life threatening illness or condition requiring EMS response. Examples include but are not limited to: loss of consciousness, intensive care treatment, emergency surgical intervention, CPR, obvious & significant blood loss. Medical emergencies. Unplanned hospitalizations. Specific diagnosis that may be covered: Sepsis, Cardiac arrest, Blood Clots, Stroke, Hypoxia, Pneumonia, Intestinal blockage, or admission to an ICU/ CCU. Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified DDSN Major Medical Other: DDSN Major Medical Other: Hospital: ER Visit without admission Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified Any visit to a Hospital Emergency Room should be documented. This includes acute medical needs, as well a ER visits for psychiatric evaluation. ER Visits Change in Medical Condition Newly diagnosed conditions, skin breakdown, infections, recurrent vomiting/diarrhea, and/or changes in behavior requiring medical care and follow-up. Out-patient services and Emergency Room visits are included in this category. Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified Other: Change in Condition

  6. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified Other: DDSN Choking Gagging or choking on food, liquid, a foreign object or material that requires intervention by medical personnel. Choking Injury: Fall or Other: Fall without injury Any fall resulting in injury requiring medical treatment beyond basic first aid. Falls resulting in a serious injury must be reported as a Critical Incident via the DDSN Incident Management System under the category of "Injury." When a plan for care and/or treatment is not followed/provided to an individual resulting in the need for medical treatment from a physician. This could include but is not limited to: personal care, hygiene, oral care, special diets/nutrition, assistive devices and/or monitoring. *Provider must also complete a Critical Incident Report on the DDSN Incident Management when there is an adverse reaction and additional medical attention is required. Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified Fall Immediate- Must be documented by the end of the shift. Medical Observed Discovered Notified Care/ Treatment Other: DDSN: Care/ Treatment The initial documentation for Choking, Falls, and Care/Treatment that was not followed according to the prescribed plan will be maintained in the GER. There will be additional prompting for the provider to complete a Critical Incident Report for these incident types.

SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Any injury, either discovered or observed, requiring medical treatment that is not considered major. (This does not include basic first aid.) Bruises, burns, bites, sprains, significant swelling and contusions are considered moderate if they require treatment beyond first aid. A serious injury either discovered or observed, requiring medical treatment, including any loss of consciousness, fracture, head injury or wound requiring more than 5 sutures/staples. Hospitalization or urgent care. *Serious injuries of Unknown Origin must be reported to SLED and the provider must follow the ANE Reporting Process. Immediate- Must be documented by the end of the shift. Observed Discovered Notified Injury: Severity- Moderate Injury: Moderate Injury Immediate- Must be documented by the end of the shift. Observed Discovered Notified Injury: Severity- Severe Injury Injury: Serious Injuries to the consumer must be documented in the GER. Minor injuries for not require a Critical Incident Report. Serious injuries will require a Critical Incident Report. Serious Injuries of Unknown Origin must be treated as an allegation of ANE.

SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER1, 2017) A medication error occurs when a recipient receives an incorrect drug, drug dose, dosage form, quantity, route, concentration, or rate of administration. A medication error is also defined as a variance of the administration of a drug on a schedule other than intended. A missed dose or a dose administered more than one hour before or more then one hour after the scheduled time constitutes a medication error. If medications are not available for dispensing, this is a medication error. Includes incidents in which the individual experienced no or minimal adverse consequences and no treatment or intervention other than monitoring or observation was Medication Immediate- Must be documented by the end of the shift in which the error was discovered. Medication Error Severity Level 1 Observed Discovered Notified Medication Error Severity:1 Medication Immediate- Must be documented by the end of the shift in which the error was discovered. Immediate- Must be documented by the end of the shift in which the error was discovered. Medication Error Severity Level 2 Includes incidents in which the individual experienced short term, reversible adverse consequences and treatment and/or interventions was/were needed in addition to monitoring and observation. Observed Discovered Notified Medication Error Severity: 2 Medication Medication Error Charting: Other: Pretreatment not provided Observation/ Pre-treatment Requirements An error occurs when a MAR is not initialed after observation/pre-treatment requirement is provided to an individual. Evidence of a documentation error is related to a specific observation/pre-treatment prescribed for collecting data to monitor medication effects. Observed Discovered Notified Medication Medication Medication Immediate- Must be documented by the end of the shift in which the error was discovered. Medication Error Severity Level 3 Observed Discovered Notified Includes incidents in which the individual experienced life-threatening and/or permanent adverse consequences. Medication Error Severity Level 3 Medication Error Charting: Other: Follow-up not provided Immediate- Must be documented by the end of the shift in which the error was discovered. Immediate- Must be documented by the end of the shift in which the error was discovered. Medical Follow-up not provided Observed Discovered Notified The person supported does not receive the prescribed medical follow-up for his/her condition resulting in a serious adverse reaction or further complications. Charting/ Documentation Error: Medication A documentation error occurs when a MAR is not initialed after medication assistance is provided to an individual. Evidence of a documentation error is denoted by blank space(s) on the MAR following the scheduled administration time of medication(s). Observed Discovered Notified Medication Error/ Charting Medication Errors will now be reported through the GER instead of paper reporting forms.

SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Death: Cause of Death Natural or Expected Immediate- Must be documented by the end of the shift. Observed Discovered Notified Death: Expected All mortalities in any setting are to be reported immediately by the provider or person notified. * Refer to DDSN Directive 505-02-DD. Death Death: Cause of Death Sudden or Unexpected Immediate- Must be documented by the end of the shift. Observed Discovered Notified Death Death: Unexpected All mortalities in any setting are to be reported immediately by the provider or person notified. *Refer to DDSN Directive 505-02-DD. A Death Report must also be completed on the DDSN Incident Management System for both Expected and Unexpected deaths. The GER in Therap may be referenced through the Therap Form ID in order to avoid duplication of effort. The IMS will require information that is not included in the GER.

  10. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) A GER is required when an individual's behavior requires the use of a manual restraint. Restrictive procedures, including Manual Restraints, should only be used in accordance with a current, approved Behavior Support Plan, except in emergency situations. All restrictive procedures must be approved through the agency's Human Rights Committee. Restraint Immediate- Must be documented by the end of the shift. Observed Discovered Notified Restraint: Manual Restraint Other: Manual A GER is required when an individual's behavior requires the use of a mechanical restraint. Restrictive procedures, including mechanical restraints, should only be used in accordance with a current, approved Behavior Support Plan, except in emergency situations. All restrictive procedures must be approved through the agency's Human Rights Committee. Restraint Immediate- Must be documented by the end of the shift. Observed Discovered Notified Restraint: Mechanical Restraint Other: Mechanical A GER is required when an individual's behavior requires the use of a chemical restraint. Restrictive procedures, including chemical restraints, should only be used in accordance with a current, approved Behavior Support Plan, except in emergency situations. All restrictive procedures must be approved through the agency's Human Rights Committee. Restraint Immediate- Must be documented by the end of the shift. Observed Discovered Notified Restraint: Chemical Restraint Other: Chemical Restrictive procedures require the approval of the provider s Human Rights Committee. Please reference DDSN Directives 600-05-DD and 567-04-DD. Any use of a restraint must be documented in Therap using the GER. If there are any injuries to he person supported, a Critical Incident Report must be completed on the DDSN Incident Management System.

  11. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Other: DDSN Law Enforcement Involvement Behavioral Immediate- Must be documented by the end of the shift. Law Observed Discovered Notified Issues Assistance/ Intervention is required from Law Enforcement and a Report/ Case ID is issued as a result of that involvement. Enforcement Involvement Other: DDSN Physical Aggression/ Assault The physical assault or physical aggression displayed between two persons supported. Includes: hitting, kicking, pinching, punching, biting, slapping, shoving or striking another. Also includes property damage. *Provider must also complete a Critical Incident Report on the DDSN Incident Management System if there are serious injuries. Behavioral Immediate- Must be documented by the end of the shift. Physical Aggression/ Assault Observed Discovered Notified Issues Behavioral Behavioral Immediate- Must be documented by the end of the shift. Observed Discovered Notified Possession or use of any firearm, weapon or explosive by a person supported. Issues Other: Weapons Weapons Immediate- Must be documented by the end of the shift. Other: DDSN Illegal Substances Observed Discovered Notified Possession or use of any illegal substances by a person supported. Issues Illegal Substances Behavioral issues may pose a risk to the person supported and/or people around them. It is important to have a record of behavior in order to ensure appropriate supports are available. The GER will be the initial source of documentation for all behavioral incidents listed above.

  12. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Provider staff must provide documentation in a GER whenever a supported person is found to have a weapon, in possession of illegal substances, or involved in sexual aggression/ assault. In addition, a Critical Incident Report will be required in the DDSN Incident Management System. Details included in the GER may be referenced in the CI Report using the Therap Form ID in order to minimize any duplication of effort.

  13. SOUTH CAROLINA DEPARTMENT OF DISABILITIES & SPECIAL NEEDS GENERAL EVENT REPORTING REQUIREMENTS (EFFECTIVE NOVEMBER 1, 2017) Users with the GER Submit role can create new incident reports and submit them for approval. They may also save partially completed reports, which will appear in the GER Worklist. Please follow the steps below to create a new GER:
