SWOCOG FSS-Respite Care w/Individual The family can add any respite provider 18 or older that does not live in the home. It cannot be a non-custodial parent. Forms needed: Provider information page W9 form Family waiver page Request for Vouchers form Once we have added the provider they remain on file from year to year. Vouchers must be requested prior to the respite care occurring and can only be used for occasional respite care. We do not cover regular weekly daycare.
SWOCOG FSS-Skilled Respite Care The family can use any skilled agency but not all will take our vouchers. Forms needed: W9 form Request for Vouchers form If the family wants to pay the skilled agency and be reimbursed, the family would fill out the W9 form. If we are paying the agency directly, the agency fills out the W9 form. Once we have added the agency they remain on file from year to year so we do have some agencies on file. Vouchers must be requested prior to the respite care occurring.
SWOCOG FSS Adaptive Equipment and Home Modifications FSS will consider any adaptive equipment or home modification that is not covered or not covered in full by the families insurance. Forms needed: Verification of Need form Letter of medical necessity from a doctor or therapist that explains the therapeutic value of the equipment or modification. Quote from the vendor that includes shipping, tax, etc. W9 form Local vendors will take our vouchers but not online or catalog companies. We can issue a check made out to the vendor as long as we have a W9 on file for them and they are willing to take a check as a form of payment. If this will not work, we can handle it as a reimbursement to the family with the family s W9 form on file. All requests have to be approved prior to making the purchase or starting the modification.
SWOCOG FSS Training/Therapy FSS will consider many types of therapy, training, and counseling as long as it if offered in a non-educational setting. Tutoring and school tuition is not covered. Forms needed: Verification of Need form Letter of medical necessity from a doctor or therapist that explains the therapeutic need and benefit. Request for Vouchers form Once a family has been approved for a specific type of therapy, we keep the documentation on file from year to year and they just need to fill out a Request for Vouchers form each year to obtain new vouchers. Vouchers must be requested prior to the therapy, training, or counseling occurring.
SWOCOG FSS Incontinence Supplies FSS will fund diapers, pullups, attends, etc. if the individual is age 3 or older. A letter of medical justification from the doctor is required. We also require proof that your insurance does not cover them before they can be approved. Forms needed: Verification of Need form Request for Vouchers form Denial from insurance or copy of policy book showing they are not a covered item. Kroger and Duraline are the companies that we have accounts with that will accept our vouchers. If the family wants to purchase diapers elsewhere, we can process the request as a reimbursement to the family with the family s W9 form. All requests have to be approved prior to making the purchase.
SWOCOG FSS Special Diets FSS will consider special diets and dietary supplements that are prescribed by a physician that are not typical formulas such as Isomil, Enfamil, etc. Forms needed: Verification of Need form Letter of medical necessity from a doctor that explains the therapeutic need and benefit. Request for Vouchers form Kroger will accept our vouchers for this as well. If the family wants to purchase the special diet items elsewhere, we can process the request as a reimbursement to the family with the family s W9 form. All requests have to be approved prior to making the purchase.
