  3. Open Call for Measure Proposals Open from May 4 to June 15 Asked that measure nominations Align with the SQAC s priorities: 1. Appropriateness of hospital-based care 2. End of life care 3. Home care 4. Integration of behavioral health and primary care 5. Long term services and supports 6. Maternity care 7. Opioid use 8. Post-acute care Fill a gap in the current SQMS 3 3

Outreach to Promote the Open Call End of Life Care: The Schwartz Center for Compassionate Healthcare Hospice & Palliative Care Federation of Massachusetts Maternity Care: Massachusetts chapter of ACOG Neonatal Quality Improvement Collaborative of Massachusetts Integration of behavioral health and primary care: Harold Pincus and Brigitta Spaeth- Rublee at Columbia University Medical Center Appropriateness of Hospital-based Care: Boston Children s Hospital via CHIA research team Opioid Use: Massachusetts Department of Public Health Professional associations and other stakeholder groups 4 4

Nominated Quality Measures Formal Survey Submissions Total of 23 nominations received 7 of these are already in the SQMS 16 new measures for consideration Nominations received from commercial payers, state agencies, providers, and researchers Recommended Next Steps SQAC staff review the 16 new measure nominations using the SQAC Measure Assessment Tool Discuss initial assessments at next meeting 6 6

Other Input We Received Staff received an additional 23 measures outside the survey process The Schwartz Center shared 8 measures for end-of-life care to consider Columbia University Medical Center shared 14 primary care/screening measures that stratify by SMI Mass Medical Society shared CAC-3: Asthmatics discharged with Home Management Plan of Care for consideration 7 7

  8. Other Input We Received GIC: Reduce number of measures that are not endorsed by key stakeholders (e.g., NQF) Outer Cape Health Services: Include screening for social determinants of health risks Tufts Health Plan: Report on existing measures by Race, Ethnicity, Language (REL) categories 8 8
