Documents Required for HR Personnel Action Request (PAR) HR 090 Updated 1/2017 Ethnic/Race Survey Updated 8/2016 Proof of NCLB (ESSA) Requirements (CODESP or Transcripts) TB Clearance Child Abuse Reporting Statement and Acknowledgement New 1/2017 Technology & Gift of Public Funds Emergency Contact Information PERS Exclusion Form Department of Justice (live scan) Clearance W-4 Electronic Deposit Authorization I-9 and copy of supporting documentation Updated 1/2017 Copy of Social Security Card (even if not used for I-9)
Changes/Updates Ethnic/Race Survey Adds additional legal questions required of all employees Mandated Reporter Acknowledgement I-9
Transmitting Information to HR All information coming to HR must first be sent through the Central Office! See handout on MiniCorp Onboarding Processes
Order of Transmission Two Packets of PDF information to be sent as attachments to email IMPORTANT As long as using BCOE email information is secure Subject labeled: Position (Name) Attachments labeled: #1 (Name); #2 (Name) Packet 1 (in this order) 1. PAR 2. Ethnic/Race Survey 3. NCIB Info (either proof of CODESP or transcripts 4. TB 5. Mandated Reporter Ack. 6. Tech Form/Gift of Public Funds 7. Emergency Contact Info 8. PERS Exclusion form Packet 2 (in this order) 1. DOJ Clearance 2. W-4 3. I-9 with copies of supporting documentation 4. Copy of Social Security card if not included as part of #3 5. Electronic Deposit Authorization (optional)
