At MRO we are often referred clients who have either been involved in a traumatic event (catastrophic motor accident) where there may have been a fatality or a significant loss (amputation / spinal cord or brain injury). The purpose of today s session is to provide an opportunity for us to share together our experiences working with clients in this space, and collate some practical suggestions on services and tips to help equip our teams on this topic. Case Study: Tim Referral received for a local family: MVA March 2021, female deceased Sarah 55 yo Tim Sarah s husband (not involved in the accident but has a claim for psychological condition) 15 yo daughter (with spinal injuries from the MVA and at Westmead Hospital, 2 month admission) who was trapped in the vehicle - has been accepted into ICARE 2 grandsons (FT custody) aged 5 and 10, one who was trapped in the MVA; the other who climbed over the deceased to exit the vehicle. Nil physical injuries but psychological symptoms (sleep/mood/travel) MRO Rounds, May 2021 - supporting clients following trauma
Issues identified at INA (2 months post MVA) Daughter was still in hospital in Sydney; boys were at Ronald McDonald house with other grandparents; husband had returned home with plans for boys to start school the next week. Nil psychological treatment had been arranged for husband or boys at time of INA Mother s day that weekend; boys returning for first time home post MVA after being in Sydney Significant change in roles: husband has never managed schooling, homework, household tasks, paying bills, internet/email MRO Rounds, May 2021 - supporting clients following trauma
Issues identified cont Nil contact had been made with the school regarding boys returning the next week House in dissarray; housework/home maintenance/yard had not been attended to Husband had # foot playing soccer 2 weeks before our assessment (in moon boot) Paperwork lost in the MVA about the custody of the boys Poor IT skills and management of financial / work / insurer matters Unclear future - ? Role of Case Manager cont Therefore, attention should be paid to social reintegration and vocational rehabilitation needs during the initial assessment and treatment planning phase. In some cases, this may include supporting the individual s capacity to stay at work or facilitating return to work as soon as is practical, even if on restricted work duties. Role of Case Manager cont The practitioner should assess immediate needs for practical and social support and provide education and referrals accordingly. Appropriate goals of treatment should be tailored to the unique circumstances and overall mental health care needs of the individual and established in collaboration with the person. PREVENTING BURN OUT compassion fatigue = repeated exposure to the traumatic experiences of others, combined with the high levels of distress often seen when people recount their experiences Self Care: skill and competency development and maintenance including regular supervision, establishing and maintaining appropriate emotional boundaries with people with PTSD, and effective self-care. Maintain a balanced and healthy lifestyle and respond early to signs of stress.
