Best Practices in Value-based Implementation Use techniques to support physicians with information, infrastructure, and incentives Make investments in culture and make them physician-led Promote transparency, co-creation, and buy-in overall Provide rewards based on short-term performance indicators Encourage selection of appropriate measures and salient incentives Provide estimates of incremental revenue that practices did not receive by not meeting targets Enable development of workflows and other tools to support clinicians in improving practice patterns Provide comparative reports between practices in the same ACO Encourage practices to focus on key upstream KPIs Proactively identify unintended consequences such as conflicts with other incentives or potential misinterpretations Prioritize high-risk patients and offer plan to close gaps Create automated work lists to coordinate outreach Create culture of change

  4. New Opportunities Covid has unlocked new opportunities particularly around telehealth and home-based care. Seizing upon these is critical.
