Demographic details : Girijjanmallappa Sajjan 70 y male Bijapur I.p 314504/2012 c/o breathlessness & breathlessness since a week Present illness : apparently well since 25 days back , insidious cough with yellow expectoration . No blood/foul smell . Ronchi present . Rest all systems normal . SOAP NOTE Subjective data : cough since 7 day with expectoration & breathlessness . Assessment Problem list : breathlessness due to inflammation of bronchioles , airway narrowing might have taken place , thus dyspnoea . Cough with expectoration : common in all respiratory impairments : it s a reflux to expel any foreign body in the respiratory tract . Acute bronchitis : is inflammation of bronchi .
