Executive Summary Personnel Resources Will the key players including employees, contractors and temporary staff with skills and experience critical to the success of your SLDS continue to be part of the program after the grant ends? Financial Support How will people, infrastructure, and other material needs be funded after the grant? Will any existing contracts extend beyond the grant period? Have you developed a budget or identified funding resources to support the SLDS? SLDS Program Priorities Without the SLDS grant project tasks, what will guide the priorities including a research agenda for the SLDS program work? Does your organization and/or the state have a strategic plan? Is the SLDS included in the plan? Governance Are changes needed to your data governance program and data sharing agreements to support the successful transition to an ongoing SLDS program? SLDS Management What organization or program area will be responsible for overseeing the daily tasks related to managing the collection of SLDS data and ensuring data quality?
