Examen clinique Percussion Palpation
Percussion Patient en d cubitus dorsal. De haut en bas: bord sup rieur De bas en haut: bord inferieur Difficult s: Distension colique, Pneumop ritoine, Pleur sie droite
Palpation D termine le bord inferieur du foie, pr cise ses caract res. Palpation unimanuelle: M thode de Chauffard Palpation bimanuelle: M thode de Gilbert M thode de Mathieu
Palpation Volume du foie: fl che h patique (10 12 cm) Le bord inf ne d borde pas le rebord costal, R gulier, lastique, surface lisse, mousse, indolore, mobile avec les mouvements respiratoires.
Diagnostic positif H patom galie diffuse: tum faction de l hypochondre droit et de l pigastre mobile avec la respiration, de situation ant rieure, dont le bord inf rieur est oblique en haut et en dedans. Ascite: signe de glacon.
Diagnostic positif H patom galie localis e: A d veloppement abdominal: tumeur de HCD ou pigastre. A d veloppement thoracique: limite sup anormalement haute, sur l vation de la coupole diaphragmatique droite.
Diagnostic diffrentiel H patom galie diffuse: Foie ptos , Foie lux (tumeur thoracique, pleur sie) Lobe d Riedel: languette qui occupe le flanc droit en continuit avec le foie.
