Akron City Campus Meds to Beds Education How the Meds to Beds Process Works
What is Meds to Beds? Instead of heading to the pharmacy after being discharged, prescriptions will be filled through the retail pharmacy on site on the ground floor near the north elevators. Once filled the finished prescription(s) can be delivered to the patient s bedside. Aside from the patient usual copay, there is no additional fee for our service Prescriptions must be paid for upon delivery via cash, credit card, check, or FSA oCredit cards can be called in by a family member if necessary The patient can now leave the hospital with all of their medications in hand. They won t need to make a separate trip to the pharmacy. 2
Who Manages This Program? The Meds to Beds program is a true multi-disciplinary, collaborative effort among numerous departments and all shifts, including medical staff, nursing, pharmacy, social work and case management, finance, technology, quality, marketing, and administration / management. Everyone that touches the patient within the program is responsible for delivering high quality care. Prescriptions that are processed within this program fall under the ultimate responsibility of Summa s pharmacy department. 3
How Does the Patient Find Out About Meds to Beds? Summa has information about the program in the patient s admission folder. Patients and family members will find there is program signage designed by Marketing throughout our facilities, including in the patient s room, to alert them of this service option. Patients can request more information about the program from the pharmacy team. A team of pharmacy assistants will be assigned to patient care areas to serve as program experts and deliver completed prescriptions back to patients. 4
Role of the Nurse Nurses should promote the Meds to Beds service to patients as a convenient solution when preparing for discharge. Nurses should understand the program well enough to address basic questions from patients and family members. Nurses will work with physicians to ensure home-going prescriptions are generated for patients ASAP, in order to give the pharmacy assistant and pharmacist time to process the order so as to not delay the discharge. All printed prescriptions must be signed by the physician. Nurses must ask the patient if they have their medications in hand prior to calling for transport at discharge Effective communication with the pharmacy assistants is crucial. 5
The Role of the Physician This program allows patients to leave the hospital with their medications, improving patient satisfaction. If you send prescriptions electronically and the pharmacy is listed as Summa Health Akron Retail Pharmacy, this means the patient would like the medications filled here before they leave. Physicians should alert the patients that they are enrolling the patients in the program and that they should understand payment is due upon delivery The sooner the pharmacy has the prescriptions, the sooner they are filled and we limit the chance of patients waiting for their prescriptions. If a clarification is needed, Meds to Beds staff will communicate with the nurses, who will then be in touch with you for the clarification.
Where Do I Obtain Help if I need it? Contact the Summa Health Akron Retail Pharmacy (Meds to Beds) at 54911 or through Vocera. Pharmacy assistants are available to answer many questions about how the program works. Pharmacists are also available for questions and consultation if needed. Unit directors and transitions of care coordinators can assist you, a patient or a family member if needed. 7

(Will show up on your dashboard) Change preferred pharmacy to Summa Health Akron Retail Pharmacy Patient decides to fill prescriptions through meds to beds When prescriptions are written: E-prescribed straight to pharmacy Signed hard copies will need to be faxed or delivered to the pharmacy Prescriptions will be delivered prior to discharge Prescriptions filled at Meds to Beds retail pharmacy 8

Changing the Patients Pharmacy In the home medications section 1. Click on the grey box that says Pharmacy 2. Click the add Pharmacy button 11
Sending Prescriptions The majority of prescriptions can be e-prescribed directly to Meds to Beds Any and all hard copy prescriptions need to be signed and delivered to the meds to beds pharmacy The prescriptions can be faxed down to the pharmacy at 330-375-4889, then placed in the patient chart. The original prescription will be collected upon delivery of medication Once sent, the prescriptions will show up on the patient s After Visit Summary If the prescriptions were sent inadvertently to another pharmacy, they can be transferred back to Summa Health Retail Pharmacy 13
Prescription Delivery The prescription will be brought to the patient s bedside and the transaction will occur in the patient s room The copay for the prescriptions must be paid at pickup and cannot be added to the hospital bill. Cash, Check, Credit Card, and FSA are accepted Please check the After Visit Summary to ensure all prescriptions are available for the patient. Do not call transport until all prescriptions are delivered At this current time, we can fill meds for bedded patients in the ER, but do not deliver to that area 14
