  2. Patient Details Name: Age: Residence: Education: Occupation: Husband: Age: Education: Occupation: SES: Mrs. X, 26 years Attebele, 12thstandard, Housewife Mr Y, 30 years Graduate, Accountant (Private company), Upper middle class

  3. Chief Complaints Excessive facial & body hair growth - 3 m Prolonged bleeding - 15 days. Giddiness 1 day

  4. Presenting illness Prolonged bleeding - 15 days. Clots + No Dysmenorrhoea Previous cycles: 6-8 / 45-60d, Irregular, Increased facial hair growth for which has taken lazer treatment. No history of similar illness in the past

  5. History of presenting illness No h/o acne, hair loss, change of voice No h/o cold intolerance, constipation No h/o nipple discharge, visual disturbances No h/o decreased libido No h/o extreme exercise

  6. Menstrual & Obstetrics History Menarche-12 years, Previous Menstrual cycles: 6-8 / 45-60 d, Irregular Using 3-4 pads per days, Not associated with clots and dysmenorrhea. Nulligravida

  7. Marital history Married since 2 years, Non consanguinous marriage Presently not planning for children.

  8. Past history Not a k/c/o hypertension, diabetes mellitus, tuberculosis, bronchial asthma, epilepsy Not a k/c/o thyroid disorders or any chronic illnesses No h/o drug allergy No h/o any surgeries in the past

  9. Family history: Both parents have diabetes mellitus well controlled on medication No history of infertility in the family (one elder brother only)

  10. Personal history Mixed diet Appetite and sleep normal Sedentary lifestyle Weight gain of 5 kgs over 6 months Normal bowel and bladder habits Coital history:Normal, not using contraception. Does not smoke, no alcohol

  11. General physical examination Patient is sitting comfortably on bed, Average built, well nourished. She is conscious, co-operative and well oriented to time, place and person Height 156cm Weight 64.5kg BMI 26.5kg/m2 Waist circumference 90cm

  12. General Physical Examination PR 88/min, regular rhythm, normal character and volume BP 122/74 mmHg, measured in left upper arm, sitting position RR 16/min, regular Temperature afebrile No pallor, icterus, cyanosis, clubbing, lymphadenopathy, pedal edema

  13. General Physical Examination Facial hair +(upper lip, chin) Modified Ferriman gallwey score - 8 (upper lip, chin, chest and abdomen) Acanthosis nigrans+ B/L breast- normal, no discharge, Tanner V Thyroid normal Spine and gait normal Scalp hair normal

  14. Systemic examination CNS no focal neurological deficits CVS No abnormality detected. RS B/L normal vesicular breath sounds heard, no added sounds

  15. Abdomen examination Inspection Abdominal obesity + Umbilicus central and inverted Coarse hair growth seen over midline from xiphisternum to pubic symphysis All quadrants move correspondingly with respiration No visible scars, sinuses or dilated veins

  16. Abdomen examination Palpation No local rise of temperature Soft, non tender No mass palpable No organomegaly Percussion Tympanic note heard all over abdomen Auscultation Normal bowel sounds heard

  17. Abdomen examination Per Speculum Examination Cervix and vagina appears healthy Minimal bleeding present from the os Bimanual Pelvic Examination Uterus is normal in size, Anteverted, Mobile, Fornices free, no mass, no tenderness. No cervical motion tenderness

  18. Summary 26 year old Housewife with AUB under evaluation She is an obese lady with prolonged cycles, excessive facial hair growth, mFG 8, no known medical/ surgical history, systemic and abdominal examination normal

  19. Provisional Diagnosis AUB under evaluation ?PCOS

  20. Investigations Urine Pregnancy Test:Negative Complete hemogram Normal OGCT: 108mg/dl TSH:3.53 IU/mL PRL:18.5 ng/ml Ultrasound:Bilateral PCOs,Uterus Normal,AF s 20-25 on both sides,ovarian volume RO: 29.6cc and LO:22.2cc.
