  1. Driving on University Business Requirements

  2. Before you start you will need your Net ID Your Net ID is a two letter four digit user ID that allows you to log into University Systems. You should have received this in an email from the University with your NET ID and Temporary Password. You must have your NET ID to access: University email (Horizon) - The University will only communicate via this account. University Student Expense Reimbursement System Required University Trainings Adobe sign system library Student travel reimbursements must be submitted on the web form CSUEB Initiate Travel Reimbursement (which would include anything travel related i.e., registration, mileage, airfare, lodging, etc)

  3. Airfare in lieu of mileage policy If an employee/student/contractor elects to drive vs flying, they may be reimbursed for the lesser amount Airfare in lieu of mileage . The employee needs to obtain a price quote from the airlines, minimum 21 day advance quote and attach it to their claim. If someone within the department is also traveling to the business conference by airlines, you may also use their ticket as a price comparison. Policy information is located on the University Travel website.

  4. Privately Owned Vehicle Operation These documents are required by any employee or student who will be driving on University business. 1. Driving Safely, Driving Smarter -Online Training 2. STD 261 -Authorization To Use Privately Owned Vehicles On State Business Form INF 1101 -Authorization of Release of Driver Record Information 3.

  5. Defensive Driving Training Step Two Step One At the end of the training you will receive a certificate. Access training via link: CSU Login Search Driving Safely, Driving Smarter in the top right-hand search bar. Once the course has been completed, Save the PDF certificate you will need this for the next step and for your Expense Claim. In the tabs row click on Courses Then click Launch to start the course.

  6. STD 261 Authorization to use Privately Owned Vehicles on State Business form This document is located on the Risk Management website under Field Trips > Transportation. You can only complete this form after you receive your certificate of completion for Driver Safety online training. Keep a copy of this for your records and submit to Accounts Payable. This document will not be used for any other purpose than the primary intent. Required, you will need to provide proof of car insurance and the insurance card must have your name.

  7. INF 1101 -Authorization of Release of Driver Record Information This document is located on the Risk Management website You can only complete this form after you receive your certificate of completion for Driver Safety online training. Keep a copy of this for your records and submit to Student Administration Building Room 2600. This document will not be used for any other purpose than the primary intent.

  8. In case of an accident While we hope your trip is a safe one we understand unforeseen events do occur. In the event of an accident you will need to follow the process detailed on the Risk Management Website > Academic Field Trip Policy and Driver Guidelines under the Transportation: Driving and Insurance drop down menu. It is important you follow the steps precisely and in a timely manner. Immediately report any bodily injury or significant property damage to the State Office of Risk and Insurance Management (ORIM) by telephone at (916) 322-0459 Weekend voicemail messages can be left at (916) 322- 8967.
