  4. Nursing Process The American Nurses Association (ANA) defined nursing as: "The diagnosis and treatment of human responses to actual or potential health problems".

  5. Benefits of Nursing Process 1. Provides an orderly & systematic method for planning & providing care. 2. Enhances nursing efficiency by standardizing nursing practice. 3. Facilitates documentation of care. 4. Provides a unity of language for the nursing. 5. Economical. 6. Increases care quality.

  6. Characteristics of the Nursing Process 1/ Within the legal of nursing. 2/ Based on knowledge. 3/ Planned-organized and systematic. 4/ Client centered. 5/ Goal directed.

  7. The Steps of the Nursing Process Steps of Nursing Process are cyclic, overlapping and interrelated: 1 Assess 2 Diagnose 3 Planning 4 Implementation 5 Evaluation.

  8. Assessment Is the most critical step. - Answers the questions: What is happening? (actual problem). What could happen? (potential problem). - Involves collecting, organizing, and analyzing information/data about the patient.

  9. Data collection & Data analysis * Types of data : There are two types of data: 1/ Subjective data. 2/ Objective data..

  10. Subjective data Subjective data consist of information that is reported by the patient and family members in health history in response to direct questioning. Subjective data are usually documented in the patient own words and include information such as sensations or emotions that only the patient can describe.

  11. Objective data Objective data are those that other members of the health care team obtain through observation, physical examination, or diagnostic testing. Objective data can be seen or measured, e.g., heart rate, wound condition, and laboratory values.

  12. Sources of data - Primary source: The patient; is always the best source. - Secondary source: Everything/everybody else.

  13. Components of the Health Assessment 1/ History - Demographic data. - CC: chief complaint. - HPI: history of present illness. - ROS: review of systems. - PMH: past medical history. - Past medical history. - FMH: family history. - Drug history. - Gynaecological history for female patient.

  14. Components of the Health Assessment 2/ Examination A/ General examination - Examination of the exposed parts of the body (head, neck, hands and feet) and examination of the vitals (Bp, pulse rate, respiratory rate and the body temperature).

  15. -General inspection: - Age - Built - Position in bed - Expression - Jaundice - Pallor - Cyanosis - Clubbing of fingers - Leg Oedema - Cervical lymphadenopathy - Examination of the head and neck - Examination of the hands and feet - Vital signs : Pulse, Temperature, Respiratory rate and Blood pressure.

  16. B/ Systematic examination Inspection. Palpation. Percussion. Auscultation.
