Overview In 2018 we told you about our 5 key work streams to deliver the Blood Health Plan These included the Appropriate Use of O D Neg work stream We identified three work stream priorities for 2018 19; these were: Develop Dashboard for individual hospitals to monitor blood use/wastage Blood Health Blood Health National Oversight Group Fate of O D Neg red cell audit with Hospital Transfusion Labs Benchmark Major Haemorrhage procedures in Wales to identify best practice So How Did We Do?

  3. WORK STREAM PRIORITY 1 Develop individual hospital & UHB Dashboards to monitor blood use/wastage

  4. Work Stream Priority 2: Where does O D Neg Go Audit Blood Health Blood Health National Oversight Group

  5. Work Stream Priority 2: Blood Stock Management Scheme (BSMS) Workshops Collaboration with BSMS to deliver expert advice on supply chain management has been undertaken with Transfusion Laboratory Managers (TLMs) across Wales The Where does O D Neg go? full audit has been completed and discussed with TLMs. A Task & Finish group has been established to write a set of All Wales best practice recommendations/standards around the supply chain management of O D Negs. This has a completion date of October 2019 Blood Health Blood Health National Oversight Group

Work Stream Priority 3: Major Haemorrhage (MH) Audit UK National Comparative Audit (NCA) on Major Haemorrhage Practice (MHP) across Health Boards in Wales has been completed and a preliminary report which has been discussed with HBs has been undertaken Blood Health Blood Health National Oversight Group The UK analysis which will allow us to benchmark practice and with hospitals across the UK is awaited

Work Stream Priority 3: Summary Audit Findings All six University Health Boards (UHBs) in Wales submitted results Blood Health Blood Health National Oversight Group 57 cases were submitted for analysis 60% 60%of patients were male 40% 40%of patients were female 20 O D Neg units were wasted 60 O D Neg units were used 12 Group specific units were 14 O D Pos units were used* wasted 138 Group specific units used *Only two UHBs used emergency O D Pos units

Work Stream Priority 3: Summary Audit Recommendations Compare the preliminary All Wales data with the NCA UK wide data to understand differences in practice and how this is managed in Wales Investigate the use of O D pos units for MH activated for males and women over childbearing age to understand why this is not as widespread and how this could be managed Discuss with hospitals/health boards the variance in practice as evidenced by this preliminary data Develop a set of guidelines/best practice advice for the management of MH incorporating relevant standards and evidence based clinical practice Blood Health Blood Health National Oversight Group

  9. Future Work O D Neg initiatives Continue work with TLMs to embed BSMS Blood supply principles into stock management. Continue to develop All Wales best practice recommendations/standards around the supply chain management of O D Negs. Audit once embedded into practice Establish MHP best practice guidance once full NCA audit data is available. Encourage use of emergency O Rh D positive red cells for males & women over child-bearing age across Wales. Audit once embedded into practice Blood Health Blood Health National Oversight Group
