US ARMY COMBAT READINESS CENTER READINESS THROUGH SAFETY Mishap Synopsis Mission: Mix materials to create a filler explosive for an improvised container. Performing duties as the class leader, the mishap student arrived late to the exercise table. When he arrived, he found his table mates actively engaged in part one of the mornings activities. The mishap student gathered the materials for part two of the morning activities and proceeded with the mixture on his own. The assigned Table Instructor briefly walked away to confer with visiting experts, leaving an uncertified Assistant Instructor to monitor the table. The mishap student followed the directions on the student mixing guide and determined he needed a larger container to efficiently grind one of the three materials. After completing this step, the mishap student added the reaming two materials to the large mortar and attempted to mix or stir the materials together. The mixing or stirring action of all three materials in the large mortar surpassed the friction threshold for the compound and in turn, initiated the detonation of the material. The exercise sheet of the student mixing guide did not identify any warnings or hazards associated with the mix, nor did it provide specific guidance on mixing procedures. The student s page labeled the mixture as an incendiary however, earlier in the mixing guide it was labeled flash powder.
