Why Shared Measures? Q: How do we know if Shared Care Committee goals and objectives are being met? A: Shared Measures: A set of processes where diverse organizations consistently gather, analyze, and report data to ensure efforts are aligned and participants hold each other accountable A set of Shared Measures related to SCC goals and objectives used cross-province to assess and compare local innovation projects and provincial initiatives
What are the Benefits? A ready-made evaluation framework A list of Shared Measures derived from your past and current Shared Care projects The ability to customize measures to your community s needs Clarity on SCC expectations for evaluation measures and processes The freedom to implement with your own evaluator Alignment of your local project goals and objectives with SC goals and objectives Greater leverage to access funding when gaps are measured across multiple communities

How Will I Use Shared Measures? Shared Measures can be used for multiple purposes including: Project Management To ensure the project is working within defined parameters Quality Improvement Continuous review and improvement of processes to ensure project management parameters, goals and objectives are being met Evaluation To determine if the project implementation is successful
Categories of Measures For Project Leads, the most relevant Shared Measures categories are outcome measures, and in some cases, process measures Process Measures Measures of specific steps that lead to an outcome Program Measures Measures that describe the projects that are being implemented Shared Measurement Policy Change Measures Measures of adoption, implementation, maintenance, or discontinuation of policies Outcome Measures Measure that reflect the impact of an intervention or treatment
Levels of Measures Formally defined by Shared Care Committee Data collected centrally, locally Data Level Used by most projects as these measures are general submitted centrally to Shared Care and analyzed for comparison across projects Example Improved patient overall satisfaction Level 1 Shared Cross- province Measures Level 2 Shared Cross- cluster Measures Level 3 Local Community Measures project clusters that focus on a theme or specialty Shared Care Committee locally centrally to Shared Care and analyzed for comparison across clusters of projects locally and provided to the Shared Care Committee in the form of project and evaluation reports Improved patient outcomes due to CBT Skills Group Number of eNotifications sent to family physicians local projects to answer specific questions that are only applicable to the project Local innovation project leaders locally

Introduction to Shared Measures Implementation The implementation of Shared Measures is in response to community requests for: Identification of specific measures to collect Clarity on SCC expectations for evaluation The freedom to work with existing evaluators The next step is piloting shared measures in two communities The pilot process will identify FAQ and concerns to address before cross-province implementation
