Preferred order for seeking Medical Treatment Employee calls UC Health, Employee Health & Wellness triage line, 513-585-8000, to discuss whether a clinic visit is warranted UC Health, Employee Health & Wellness Clinic Open weekdays 7:30 am to 4 pm 513-585-6600 3200 Burnet Avenue, South Entrance The University Hospital Emergency Room For emergency treatment and after hours services The nearest Medical Facility All other times and holidays
Incident Reporting Complete the Initial Report on Work-related Injury or Illness, form A1352a, within 24 hours The injured employees answers the questions The supervisor checks for completion The supervisor signs the report To distribute the report: Mail the original to ML 0218 Fax a copy to 513-558-0676 Give a copy to the injured employee Give a copy to your Business Office
Time Off and Returning to Work The employee notifies the supervisor of absence daily The supervisor or departmental designee coordinates return to work whenever the employee is: Absent more than one full day of work Released with restrictions The supervisor schedules a return-to-work evaluation at UC Health, 513-585-6600, for the day the employee is to return to work
Filing Workers Comp. Claims Compensation may be provided for payment of medical bills and/or for lost time compensation Submit a Bureau of Workers Compensation application on the FROI-1 form: First Report of Injury Call 513-556-6381 for a copy of the FROI-1 form or access it on-line at https://info.bwc.ohio.gov/wps/portal/gov/bwc
Compensation for Lost Wages Direct the injured employee to contact Human Resources at 513-556-6381 for details before making the decision to apply for compensation for lost wages The employee should consult with Human Resources before deciding whether or not to use sick time Payments are provided by the Bureau of Workers Compensation, not the University
Accident Assessment and Corrective Actions Environmental Health and Safety works with the employee, supervisor, and the department to: Identify the cause of the problem Correct the cause of the problem Determine the severity of the injury or illness: The extent of medical treatment The number of missed workdays The number of restricted workdays

What should I do to reduce risk? Immediately seek help if a spill occurs View the SDS before using a chemical Properly dispose of hazardous chemical and infectious waste Ensure emergency equipment is always accessible
How do I reduce the risk for sprains, strains and other ergonomic injuries? Ask for help in resolving ergonomic problems. Call EH&S at 556-4968 today Provide ergonomic chairs, stools, and desks Place work supplies and equipment within comfortable reach Purchase a cart or dolly, eliminate manual material handling tasks Store heavy materials at waist level
