Definition of the wound Wound ; Is a break in the normal integrity of the skin and tissue Purpose of wound Dressing 1- TO protect the wound from infection 2- TO protect the wound from mechanical injury 3- TO provide proper temp 4- TO provide proper humidity 5- Proper body image 6- Enhance wound healing 7- Wound assessment 8- TO absorb exudates M.Sc Hassanain Mohammed Kadhim

Wound Assessment 1- Location 2- Color ;- pink ----normal -Red -----inflammation -Cyanosis or black ------abnormal necrosis or damage in the tissue 3- Size ;length , deep, width 4- Cleanliness ; - clean dry wound dirty dry wound - 5- Exudates according to COLDSPA 6- Pain Assessment - - M.Sc Hassanain Mohammed Kadhim

Exudates Exudates assessment (( amount , color , Odor ,consistency )) 1- Serous ((plasma )) it is a type of exudates yellow in color clear watery with out odor . 2- Sanguineous (( blood )) (( Frank & bright)) it is a type of exudates consist a large number of red blood cells red in color 3- Purulent (( pus )); it is a type of exudates yellow to green in color thick , bad in Odor consist of WBC & dead tissue & BOTH dead & live cells There are 4 type :

  5. Exudates 4- Serosanguineous : it is a type of exudates is a mixture of serum and red blood cells , light pink in color Type of Dressing 1- Dry to Dry 2- Dry to Wet 3- Wet to Wet M.Sc Hassanain Mohammed Kadhim

  6. Equipment for dressing 1- water proof drape ----- TO maintain the bed & clothes of the patient clean 2- clean gloves -----TO remove the old dressing 3- sterile gloves ---- TO performed the dressing 4- normal saline 5- Iodine M.Sc Hassanain Mohammed Kadhim

  8. Surgical Complications 1- Wound Dehiscence and Evisceration Wounddehiscence (disruption of surgical incision or wound). Wound evisceration (protrusion of wound contents) . 2- Infection 3-Hematoma Bleeding occurs under the skin at the surgical site M.Sc Hassanain Mohammed Kadhim
