Program Design $25 Incentive 2024 WELLNESS PROGRAM Eligibility: Employees + their covered spouse/domestic partner enrolled in a WCIF medical plan. Dependent children are not eligible to participate. Rewards Points: Immediate upon completion, with the exception of WCIF- sponsored contest completions. Points are awarded roughly two weeks after a contest ends. Rewards Delivery: The member will receive their reward(s) from Tango roughly two weeks upon receiving credit for completion of each $25 reward earned, up to $75 total. 05 PAGE x PAGE x Page 5
Program Design $25 Incentive 2025 WELLNESS PROGRAM UPDATE Participation Reporting: Reports will only be available once per month beginning in 2025. Other Wellness Opportunities REMINDERS Wellness Program Deadline: All activities must be completed by October 31, 2024. Wellness Champion Elections: Information will be emailed in October. Elect your group s Wellness Champion(s) by November 30, 2024. Wellness Grant Program Use of Grant Funds & Request for Reimbursement: Funds must be utilized and requests for reimbursement must be submitted by November 30, 2024. Any items purchased or forms submitted after this date will not qualify for reimbursement. 2025 Wellness Grant Program: Employer groups can begin applying for a 2025 grant December 1, 2024.
