SWAN model of care Sign is the patient believed to be entering the dying phase of life; start the individual plan of care and support for the dying person Words sensitively communicate with the patient and those important to the patient Needs are the needs of the patient being met, documented and reviewed regularly Actions step outside the box and facilitate what is important to the patient
Aims In Roman times a Swan was a symbol of a happy and good death Making End of Life Care everyone's business Creating a culture of openness at End of Life Facilitating choice Joining up EOLC and Bereavement care Making a difference to the patient and those that they love
