THIS IS ME I have been a Behavioral Health Consultant since September 2017 I have been a BHC at FHS since August 2021 My previous experience was in Kansas City, MO I am the Lead for Integrative Behavioral Health Program I did not have previous professional experience with SUD or MAT I have participated with I-ROPPES since May 2022
FHS identified: 1.Chronic Non-Malignant Pain 2.Diabetes with an A1c over 9 3.Anxiety with a GAD-7 over 15 4.Depression with a PHQ-9 over 15 BHC STANDING ORDERS 5.MAT 6.SBIRT Based on the PCBH Model, FHS Administration created standing orders for BHCs to automatically work with patients. 7.SBQ-R THIS TRAINING IS SPECIFIC TO MAT AND SBIRT STANDING ORDERS
BHC WORKFLOW FOR MAT PATIENTS 1. BHC INTERVENTIONS There are SEVERAL ways to address OUD in the Primary Care Setting. At FHS, we use a standard SBIRT Model of treatment (Screening, Brief Intervention, and Referral to Treatment) Here are a few BHC Brief Interventions I use: 1. Behavioral Activation and Modification 2. CBT 3. FACT 4. Harm Reduction 5. Motivational Interviewing
