Workplace based Monitored Sign offs (Mandatory) Workplace based Monitored Sign offs (Mandatory) Attending MDT: Attending MDT: This can be an MDT based on their clinical placement, i.e. Stroke MDT, Lung MDT, Cancer MDT, Frailty MDT Write reflection in the portfolio on the event Patient case presentation: Patient case presentation: Sites to assign F1 and above resident doctors to engage the students and allocate patient to review and later present to them. The resident doctors can sign off Clinic attendance 1: Clinic attendance 1: With a focus on chronic illness. Examples: PD clinic, MS clinic, Falls clinic, Renal clinic, Frailty clinic Can be consultant/ Registrar or Specialist nurse lead clinics Clinic attendance 2: Clinic attendance 2: With a focus on chronic illness. Examples: PD clinic, MS clinic, Falls clinic, Renal clinic, Frailty clinic Can be consultant/ Registrar or Specialist nurse lead clinics Additional option instead of second clinic sign off Additional option instead of second clinic sign off Patient hospital experience: Patient hospital experience: Students can accompany parents to PT, OT, SLT sessions, Dialysis sessions, Chemotherapy sessions. Students will be able to explore their views on the service provided, challenges faced, anxieties and expectations. CS can sign off
