  2. Why is a Keyworker Service being developed? The Keyworking Function has been developed as a response to the Long-Term Plan commitment that by 2023/24, young people & adults with a learning disability, autism or both, with the most complex needs will have a designated Keyworker. This offer extends to those who face multiple vulnerabilities such as Children looked after and adopted children, and those Young People in transition between children & adult services. In the first phase the focus was on Children and young people at risk of admission, or already an inpatient in a CAMHS hospital. We are now commencing a graded approach to 18-25 year olds. A key aspect of the Keyworking Function is ensuring the implementation of Care and Treatment Review recommendations. This is to ensure timely hospital discharges, prevent admissions and provide effective transition support. The Keyworking model has been developed through extensive consultation with young people, adults, parent carers and other stakeholders at both a National and a local Level.

  5. Role of a Keyworker Main point of contact for children and young people and their family. Build and maintain positive, trusting, and appropriate relationships with children and young people and their families. Work on behalf of children and young people and their family/carers towards unblocking challenges and barriers. Ensure all children/young people have a clear personalised plan to promote their development. To ensure that Care Education and Treatment Review recommendations/ actions are completed in a timely manner and that there are updates to the Dynamic Support Register to reflect this. Continuity of care for the child/young person, and work across systems and services to ensure they respond and react to child/young person s needs as required.

  6. Role of a Keyworker

  7. Eligibility Criteria Children and young people with a diagnosis of learning disability autism or both. The primary focus of the service has been for children and young people under the age of 18. Potential risk of admission (RAG rated red on the DSR) to a CAMHS inpatient bed or are currently in a CAMHS inpatient bed. In the future the offer will be extended to young adults up to the age of 25 years. Initially the child/young person is on the Dynamic Support Register and at the Red or amber level of the register. The home address is within the geographical area of Humber and North Yorkshire, however if an inpatient stay is outside of this the Keyworker will continue to support to bring consistency and local connection enabling contact with the family and the team around the child at place.

Children and Young Persons Dynamic Support Register (DSR) To monitor Children/young people with a diagnosis of Learning Disability, Autism or both who have complex needs and may be at risk of admission to hospital or placement breakdown. To identify Children/young people who may require a Care education and Treatment review (CETR). To support a needs led, personalised approach to care. To support services to be pro-active and ensure a multi-agency approach to care. Risk stratification.
